การตั้งครรภ์ในวัยรุ่น หมายถึง การตั้งครรภ์ในสตรีที่มีอายุระหว่าง 10-19 ปี โดยถืออายุ ณ เวลาที่คลอดบุตร ซึ่งอาจแบ่งได้เป็น adolescents คือช่วงอายุ 15-19 ปี และ younger adolescents คือช่วงอายุ 10-14 ปี1
อุบัติการณ์
ในแต่ละปีมีสตรีวัยรุ่นคลอดบุตรทั่วโลกราว 14 ล้านคน โดยประมาณ 12.8 ล้านคน หรือมากกว่า 90% อยู่ในกลุ่มประเทศกำลังพัฒนา2
อัตราคลอดในวัยรุ่นอายุ 15-19 ปี เฉลี่ยทั่วโลกประมาณ 65 รายต่อสตรีวัยรุ่น 1,000 ราย สูงสุดในกลุ่มประเทศแถบแอฟริกาใต้ บางประเทศในเอเชียใต้ และลาตินอเมริกา รองลงมาคือ ตะวันออกกลาง, แอฟริกาเหนือ, สหรัฐอเมริกา และยุโรปตะวันออก ส่วนประเทศที่มีอัตราคลอดในสตรีวัยรุ่นต่ำสุดคือ ประเทศแถบสแกนดิเนเวีย, สวิตเซอร์แลนด์, ญี่ปุ่น, เกาหลี และจีน
ความดันโลหิตสูงในขณะตั้งครรภ์ (Hypertensive disorders)
International Society for the Study of Hypertension in Pregnancy (ISSHP) ได้ให้คำจำกัดความของความดันโลหิตสูงในขณะตั้งครรภ์ ว่า ระดับ diastolic blood pressure ³90 mmHg ในช่วงครึ่งหลังของการตั้งครรภ์ ส่วนครรภ์เป็นพิษคือภาวะความดันโลหิตสูงร่วมกับการพบโปรตีนในปัสสาวะ จากการศึกษาส่วนใหญ่พบว่าอุบัติการณ์ของภาวะความดันโลหิตสูงในขณะตั้งครรภ์ในวัยรุ่นไม่ได้แตกต่างกับในผู้ใหญ่ แต่บางรายงานพบว่ามีอุบัติการณ์ที่สูงขึ้น1 สำหรับในประเทศไทยพบว่าอุบัติการณ์ของความดันโลหิตสูงในขณะตั้งครรภ์ของวัยรุ่นไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติเมื่อเทียบกับในผู้ใหญ่6-10
ภาวะโลหิตจาง (Anemia)
ในการตั้งครรภ์ปกติพบว่าปริมาตรของพลาสมาเพิ่มขึ้น ทำให้ความเข้มข้นของฮีโมโกลบิน และฮีมา-โตคริตลดลง เกิดภาวะโลหิตจางได้ องค์การอนามัยโลกให้คำจำกัดความของภาวะโลหิตจางในขณะตั้งครรภ์เมื่อฮีโมโกลบินต่ำกว่า 11.0 g/dl ฮีมาโตคริตต่ำกว่า 31% ซึ่งในภาวะ hemodilution ฮีโมโกลบินอาจต่ำได้ถึง 10.4 g/dl ส่วนภาวะโลหิตจางรุนแรงนั้นคือฮีโมโกลบินต่ำกว่า 7.0 g/dl สำหรับในประเทศที่พัฒนาแล้วพบว่าอุบัติการณ์ของภาวะโลหิตจางในหญิงตั้งครรภ์วัยรุ่นเพิ่มขึ้นโดยมี relative risk ประมาณ 2 เท่าบางรายงานพบอุบัติการณ์ไม่ต่างกัน สำหรับประเทศกำลังพัฒนาพบว่ามีอุบัติการณ์ของภาวะโลหิตจางในทั้งสองกลุ่มเท่าๆกัน แต่ความรุนแรงต่างกัน เนื่องจากในกลุ่มประเทศเหล่านี้ ภาวะโลหิตจางเป็นปัญหาสำคัญของหญิงตั้งครรภ์ในทุกกลุ่มอายุ ซึ่งสาเหตุเกิดจากภาวะทุพโภชนาการ, การขาดธาตุเหล็ก และโฟเลต, มาลาเรีย, พยาธิ, โรคทางพันธุกรรม (Sickle cell anemia, Thalassemia) ซึ่งภาวะโลหิตจางสามารถรักษาได้ในช่วงของการฝากครรภ์ แต่เนื่องจากสตรีวัยรุ่นมักฝากครรภ์ช้า จึงทำให้พบอุบัติการณ์ของภาวะโลหิตจางได้มากกว่า1 ในประเทศไทยพบภาวะโลหิตจางในหญิงตั้งครรภ์วัยรุ่นสูงขึ้นเล็กน้อย แต่มีบางรายงานพบว่าไม่มีความแตกต่างกัน6,8,10
มาลาเรีย1
ในหญิงตั้งครรภ์พบปริมาณของเชื้อในกระแสเลือดและความรุนแรงของโรคมากกว่าสตรีที่ไม่ได้ตั้งครรภ์ นอกจากนี้ยังพบว่ามีการติดเชื้อที่รกได้ ซึ่งส่งผลกระทบต่อทั้งมารดาและทารก พบว่าในหญิงตั้งครรภ์ท้องแรกมีความเสี่ยงต่อการติดเชื้อได้มากกว่าหญิงตั้งครรภ์ท้องหลัง และเนื่องจากหญิงตั้งครรภ์วัยรุ่นมารับการฝากครรภ์ได้ไม่เพียงพอ จึงทำให้การควบคุมโรคและการรักษาเป็นไปได้ไม่ดีเท่าที่ควร
การติดเชื้อเอชไอวี และโรคติดต่อทางเพศสัมพันธ์อื่นๆ
การติดเชื้อเอชไอวีมีความสำคัญในหญิงตั้งครรภ์ เนื่องจากสามารถถ่ายทอดจากมารดาสู่ทารกได้ ในประเทศที่พัฒนาแล้ว พบอุบัติการณ์การถ่ายทอดจากมารดาสู่ทารกในหญิงตั้งครรภ์ที่ไม่ได้ให้นมบุตร 14-25% แต่สำหรับในรายที่ให้นมบุตรพบอุบัติการณ์ 13-42% แต่เมื่อได้มีการให้ยาต้านไวรัสในสตรีตั้งครรภ์แล้วพบอุบัติการณ์ของการติดเชื้อในทารกลดลง ในสหรัฐอเมริกาพบว่าลดลงจาก 22.7% เป็น 3.3%1
สำหรับในสตรีวัยรุ่นนั้น เนื่องจากเป็นวัยที่มีพฤติกรรมทางเพศสม่ำเสมอ ทำให้มีความเสี่ยงต่อการติดเชื้อเอชไอวี หรือโรคติดต่อทางเพศสัมพันธ์อื่นๆ ได้มากขึ้น จากการศึกษาในประเทศไทย พบว่าอุบัติการณ์การติดเชื้อในหญิงตั้งครรภ์วัยรุ่นไม่แตกต่างกับในผู้ใหญ่9,10
ภาวะขาดไอโอดีน (Iodine deficiency)1
ภาวะขาดไอโอดีนทำให้เกิดความบกพร่องทางพัฒนาการทั้งทางด้านร่างกายและสติปัญญา ในช่วงตั้งครรภ์หากขาดไอโอดีนจะทำให้สมองของทารกถูกทำลาย ทำให้สติปัญญาบกพร่องและเกิดโรคทางระบบประสาทอื่นๆ ได้ องค์การอนามัยโลกจึงแนะนำให้มีการเสริมไอโอดีนให้กับหญิงวัยเจริญพันธุ์ทุกคน โดยเฉพาะในพื้นที่ที่ขาดไอโอดีน
การเจริญเติบโตระหว่างตั้งครรภ์และกระดูกเชิงกรานเจริญไม่สมบูรณ์ (Growth during pregnancy and immaturity of the pelvic bone)1
ในกลุ่มวัยรุ่นอายุน้อยยังอาจมีการเจริญเติบโตทางด้านร่างกายได้ในขณะตั้งครรภ์ โดยอาจมีความสูงเพิ่มขึ้นได้ 2-16 เซนติเมตร นับตั้งแต่ฝากครรภ์ครั้งแรกจนถึงหลังคลอด
ในประเทศพัฒนาแล้ว พบว่าช่วงที่ความสูงเพิ่มขึ้นมากที่สุด คือช่วงอายุ 12 ปี, ประจำเดือนครั้งแรก อายุ 13 ปี หลังจากนั้นยังมีการเจริญในด้านความสูงต่อไปจนกระทั่งหลังอายุ 15 ปี ในประเทศกำลังพัฒนา อายุเฉลี่ยของการมีประจำเดือนครั้งแรกคือ 14 ปี ซึ่งช้ากว่าในประเทศพัฒนาแล้ว และพบว่าการเจริญของช่องทางคลอดช้ากว่าการเจริญในด้านความสูง โดยจะเจริญหลังจากการมีประจำเดือนครั้งแรกไปแล้ว 1-2 ปี และยังเจริญต่อไปแม้ส่วนสูงจะคงที่แล้ว
จากการศึกษาในประเทศไนจีเรีย พบอุบัติการณ์ของเชิงกรานแคบ ในหญิงตั้งครรภ์วัยรุ่นอายุน้อย คืออายุน้อยกว่า 16 ปี และส่วนสูงน้อยกว่า 150 เซนติเมตร แต่ในสหรัฐอเมริกา การศึกษาในสตรีตั้งครรภ์อายุน้อยกว่า 16 ปี พบว่าอัตราการผ่าตัดคลอดในกลุ่มนี้น้อยกว่าในผู้ใหญ่ และอัตราการผ่าตัดคลอดไม่ได้ขึ้นกับอายุกระดูก
