6. สำเนาใบอนุญาตทำงานของกรรมการผู้มีอำนาจลงชื่อผูกพันบริษัทที่เป็นคนต่างด้าว กรณี
กรรมการผู้มีอำนาจลงชื่อผูกพันบริษัทไม่ได้ทำงานในประเทศไทยและไม่มีใบอนุญาตทำ
งาน ต้องให้ Notary Public รับรอง และทำหนังสือมอบอำนาจให้กรรมการ หรือบุคคลใด

7. สำเนาการศึกษาและหลักฐานการผ่านงาน (กรณีที่ไม่สามารถได้ ให้ใช้แบบฟอร์มตามที่
