อนึ่ง ในขณะที่นักศึกษาฝึกงานอยู่ในสถานประกอบการของท่าน ทางภาควิชา ฯ ได้มอบหมายให้อาจารย์ของภาควิชา ฯ มาคอยให้คำปรึกษา โดยจะเข้ามาติดต่อประสานงานกับท่านในภายหลัง และเมื่อนักศึกษาได้ทำการฝึกงานเสร็จสิ้นแล้วขอได้โปรดมอบหมายให้ผู้เกี่ยวข้องประเมินผลการฝึกงานของนักศึกษาลงในแบบประเมินที่แนบมานี้ด้วย พร้อมทั้งส่งคืนทางไปรษณีย์หรือจะฝากไปกับนักศึกษาตามแต่จะเห็นสมควร
