2. กะกลางคืน ขาดคนประเมินสถานการณ์ฉุกเฉินกรณีเกิดเหตุอัคคีภัย ทำให้ รปภ. ไม่สามารถตัดสินใจได้ว่าต้องฟัง
คำสั่งใคร เพื่อแจ้งขอความช่วยเหลือจากหน่วยงานภายนอก เช่น สถานีดับเพลิง
3. ผังอพยพหนีไฟ บริเวณห้องเก็บสารเคมี ไม่มีการแสดงที่ตั้งของฝักบัวฉุกเฉิน
