ทั้งสองฝ่ายตกลงทำสัญญา มีข้อความดังต่อไปนี้

เมื่อสัญญาฉบับนี้ครบอายุลง แล้วผู้ว่าจ้างยังยินยอมให้ผู้รับจ้างทำงานต่อไป ให้ถือว่าสัญญานี้มีผลต่อไปอีกจนกว่าผู้ว่าจ้างจะบอกเลิกสัญญาโดยมีข้อกำหนดและเงื่อนไขเช่นเดียวกับสัญญานี้ทุกประการ
ข้อ 3 คู่สัญญาตกลงค่าจ้างกันเป็นเงินเดือนละ ___________ บาท (_____________________________________)

โดยผู้ว่าจ้างจะจ่ายเงินค่าจ้างให้แก่ผู้รับจ้างภายในวันที่ __1-5__ของทุกเดือนที่ผู้รับจ้างจะเข้าไปดำเนินการทำความสะอาดนั้น โดยโอนเงินค่าจ้างให้แก่ผู้รับจ้างไปยัง บัญชีธนาคาร กสิกรไทย สาขา หัวหินมาร์เก็ตวิลเลจ ประเภทบัญชี ออมทรัพย์ ชื่อบัญชี นางสาวธนวรรณย์ พิมพ์ประจิตร์ เลขที่บัญชี 485-2-41126-6 โดยผู้รับจ้างไม่มีนโยบายให้มีการจ่ายค่าจ้างโดยตรงกับพนักงานที่เข้าไปทำความสะอาดให้กับผู้ว่าจ้างโดยเด็ดขาด
ข้อ 4 ผู้รับจ้างได้ให้สัญญาคู่ฉบับ สำเนาบัตรประจำตัวประชาชนของผู้รับจ้าง และพนักงานของผู้รับจ้างไว้ให้กับผู้ว่าจ้างยึดถือไว้แล้วในวันทำสัญญานี้ หากผู้รับจ้างไม่ปฏิบัติตามสัญญาแม้ข้อหนึ่งข้อใด ผู้ว่าจ้างมีสิทธิจะยกเลิกสัญญาการว่าจ้างได้ โดยผู้ว่าจ้างจะทำการแจ้งให้ผู้รับจ้างรับทราบล่วงหน้าอย่างน้อย__1__สัปดาห์ก่อนกำหนดการเข้าไปให้บริการทำความสะอาดครั้งต่อไป

ข้อ 5 หากผู้ว่าจ้างมีเหตุจำเป็นต้องเลื่อนกำหนดหรือยกเลิกเข้าไปปฏิบัติงานทำความสะอาดชั่วคราว ผู้ว่าจ้างจะทำการแจ้งให้ผู้รับจ้างรับทราบล่วงหน้า หรืออย่างช้าที่สุด __2__ วันก่อนกำหนดวันเข้าปฏิบัติงาน หากผู้ว่าจ้างไม่ทำการแจ้งล่วงหน้า และเกิดกรณีพนักงานของผู้รับจ้างไปรอปฏิบัติงานยังสถานที่ของผู้ว่าจ้างแล้ว ผู้รับจ้างจะถือว่าได้มีการเข้าปฏิบัติงานแล้ว โดยจะยังคงคิดค่าจ้างเต็มจำนวนตามปกติ และไม่มีการหักชดเชยคืนให้แก่ผู้ว่าจ้าง ถึงแม้พนักงานของผู้รับจ้างจะไม่ได้เข้าไปดำเนินการทำความสะอาดให้กับผู้ว่าจ้างก็ตาม
ข้อ 6 หากพนักงานของผู้รับจ้างมาปฏิบัติงานไม่สามารถเข้าไปปฏิบัติงานได้ ผู้รับจ้างยินยอมให้ผู้ว่าจ้างหักเป็นเงินชดเชยการทำงานหรือการขาดจำนวนของพนักงานในอัตราเฉลี่ยต่อวัน ในกรณีที่ผู้ว่าจ้างขอให้ผู้รับจ้างเพิ่มจำนวนพนักงานจากที่กำหนดไว้ใน บัญชี ก ผู้ว่าจ้างตกลงเพิ่มค่าจ้างสำหรับพนักงานทำความสะอาดคนละ _______________ บาทต่อเดือน

ข้อ 8 หากพนักงานของผู้รับจ้างก่อให้เกิดความเสียหายต่อทรัพย์สิน หรืออันตรายต่อบุคคลหนึ่ง บุคคลใด ผู้รับจ้างยินยอมรับผิดชดใช้ให้จนครบ ในกรณีที่ทรัพย์สินของผู้รับจ้างได้รับความเสียหายโดยมิใช่เป็นผลมาจากการกระทำของผู้ว่าจ้างหรือพนักงานของผู้ว่าจ้าง ผู้ว่าจ้างไม่ต้องรับผิดชดใช้ให้แก่ผู้รับจ้างหรือพนักงานนั้นแต่อย่างใด
ข้อ 9 หากผู้ว่าจ้างหรือตัวแทนของผู้ว่าจ้างพบว่าพนักงานของผู้รับจ้างคนใดปฏิบัติผิดระเบียบที่ผู้ว่าจ้างกำหนดไว้ หรือประพฤติตนไม่เหมาะสม หรือไม่มีความสามารถ ฯลฯ และได้แจ้งให้ผู้รับจ้างเปลี่ยนพนักงานผู้นั้นแล้ว ผู้รับจ้างต้องเปลี่ยนพนักงานผู้นั้นไม่ให้ปฏิบัติหน้าที่ในสถานที่ของผู้ว่าจ้างอีกต่อไปแล้วมอบหมายให้พนักงานคนอื่นมาทำงานแทน
ข้อ 10 ผู้ว่าจ้างหรือตัวแทนจะต้องทำการตรวจงาน หากพบว่าสะอาดเรียบร้อยดีแล้วจึงลงชื่อรับมอบงานให้กับพนักงานของผู้รับจ้าง แต่หากยังไม่สะอาดเรียบร้อย พนักงานของผู้รับจ้างจะต้องแก้ไขให้สะอาดเรียบร้อยโดยผู้รับจ้างจะไม่คิดค่าจ้างเพิ่มเติมจากกรณีการแก้ไขงานให้เรียบร้อยนั้น
ข้อ 11 หากน้ำยา หรือ อุปกรณ์ไม่มีเพียงพอต่อการปฏิบัติงาน หรืออุปกรณ์ใดๆ มีความผิดปกติ พนักงานผู้รับจ้างจะต้องทำการแจ้งให้กับผู้ว่าจ้างรับทราบเพื่อจัดเตรียมให้มีพร้อมสำหรับการปฏิบัติงานต่อไป หากเครื่องมือ อุปกรณ์ และวัสดุ ชำรุดและต้องเปลี่ยนใหม่ ผู้ว่าจ้างจะทำการเปลี่ยน และต้องให้คงจำนวนของเครื่องมือ อุปกรณ์ และวัสดุ ตามที่ระบุไว้ในบัญชี ข ตลอดระยะเวลาของสัญญานี้

ข้อ 13 หากปรากฏว่าผู้รับจ้างไม่ปฏิบัติตามให้เป็นไปตามที่เงื่อนไขแห่งสัญญาข้อหนึ่งข้อใดก็ตาม หรือมีเหตุที่ชี้ให้เห็นว่าผู้รับจ้างไม่สามารถทำงานตามสัญญานี้ให้แล้วเสร็จเรียบร้อย รวมตลอดถึงคุณภาพของงานไม่เป็นไปตามที่ผู้ว่าจ้างกำหนด ผู้ว่าจ้างมีสิทธิบอกเลิกสัญญาโดยแจ้งเป็นลายลักษณ์อักษรให้ผู้รับจ้างทราบล่วงหน้าไม่น้อยกว่า __7__ วัน โดยผู้รับจ้างไม่มีสิทธิเรียกร้องค่าเสียหายใด ๆ ทั้งสิ้น
ข้อ 14 ผู้รับจ้างสัญญาว่าจะไม่เอางานตามสัญญานี้ทั้งหมดหรือส่วนใดส่วนหนึ่งไปให้ผู้อื่นรับจ้างช่วงโดยไม่ได้รับอนุญาตเป็นหนังสือจากผู้ว่าจ้างก่อน และหากผู้ว่าจ้างอนุญาตผู้รับจ้างยังคงต้องรับผิดชอบตามสัญญานี้ทุกประการ
