ข้าพเจ้า นายสมศักดิ์ นะวิโรจน์ ตำแหน่ง........................... บริษัท เอเซียกังนัม จำกัด(ผู้ว่าจ้าง) ขอรับรองว่าบริษัท ไทตั้น เอ็นจิเนียริ่ง แอนด์ คอนซัลแตนท์ จำกัด 718/4 ซอยสุขุมวิท 105 ถนนลาซาล แขวงบางนา เขตบางนา กรุงเทพมหานคร(ผู้รับจ้าง) ได้ทำการจัดหาวัสดุ อุปกรณ์ในการปรับปรุง SUBSTATION 115 KV ณ บริษัท เอเซียกังนัม จำกัด เป็นราคาค่าจ้างรวมภาษีมูลค่าเพิ่มแล้วทั้งสิ้น 1,455,200 บาท ( หนึ่งล้านสี่แสนห้าหมื่นห้าพันสองร้อยบาทถ้วน ) และได้ทำการติดตั้งวัสดุ อุปกรณ์ในการปรับปรุง SUBSTATION 115 KV ณ บริษัท เอเซียกังนัม จำกัด เป็นราคาค่าจ้างรวมภาษีมูลค่าเพิ่มแล้วทั้งสิ้น 807,850 บาท ( แปดแสนเจ็ดพันแปดร้อยห้าสิบบาทถ้วน ) ตามหนังสือสัญญาเลขที่ ฉช.018/57 และ ฉช.019/57 ลงวันที่ 6 มกราคม 2558 ซึ่งงานดังกล่าวได้เสร็จสมบูรณ์เป็นที่เรียบร้อยตามสัญญาทุกประการ จึงออกหนังสือรับรองให้ไว้เป็นสำคัญ.
