โดยหนังสือฉบับนี้ ข้าพเจ้า บริษัท (“บริษัท”) สำนักงานใหญ่ ตั้งอยู่เลขที่ 252/35-36 อาคารเมืองไทย ภัทร คอมเพล็กซ์ อาคาร 1 ชั้น 27 - 28 ถนนรัชดาภิเษก แขวงห้วยขวาง เขตห้วยขวาง กรุงเทพมหานคร 10310 โดย กรรมการผู้มีอำนาจกระทำการแทน ซึ่งต่อไปนี้จะเรียกว่า “ ผู้มอบอำนาจ” ขอเพิ่มเติมรายนามผู้รับมอบอำนาจนอกจากที่ระบุไว้แล้วในหนังสือมอบอำนาจฉบับลงวันที่ 11 มิถุนายน 2555 คือ



เป็นผู้มีอำนาจกระทำการแทนผู้มอบอำนาจในกิจการทั้งปวง เกี่ยวกับธุรกรรมทางการเงินของผู้มอบอำนาจที่มีอยู่กับ ธนาคารกสิกรไทย จำกัด(มหาชน) (“ธนาคาร”) เช่น การสั่งจ่ายเช็ค การลงนามในตั๋วสัญญาใช้เงิน การเปิดแอลซี การซื้อเงินตราต่างประเทศล่วงหน้า ลงนามในเอกสารที่เกี่ยวข้องกับการทำธุรกรรมเงินตราต่างประเทศ ซื้อสมุดเช็ค ฝาก ถอน ออกเช็ค ลงชื่อเป็นผู้สั่งจ่าย หรือสลักหลังในเช็คหรือตั๋วเงินที่จะพึงมีเกี่ยวกับบัญชีของผู้มอบอำนาจ (ตามตัวอย่างลายมือชื่อข้างท้ายนี้ หรือตามตัวอย่างลายมือชื่อที่มอบไว้กับธนาคาร) ลงชื่อรับรองยอดเงิน และเอกสารหรือสัญญาทางการเงินทุกชนิดที่บริษัทมีอยู่กับธนาคารตามที่ได้รับจากกรรมการบริหารหรือกรรมการบริษัทแล้ว โดยการลงนามเพื่อทำนิติกรรมดังกล่าวให้กระทำภายใต้เงื่อนไขดังนี้
1) ผู้รับมอบอำนาจจำนวน 2 คน ลงนามร่วมกันสามารถทำธุรกรรมไม่จำกัดวงเงิน
2) การสั่งจ่ายเงินจากบัญชีกระแสรายวันให้มีข้อความ “เพื่อและในนามบริษัท” พิมพ์อยู่ในเช็คโดยไม่ต้องลงตราประทับของบริษัท
3) การสั่งจ่ายเงินจากบัญชีออมทรัพย์ และการลงนามในเอกสารทางการเงินอื่นๆ ให้ผู้รับมอบอำนาจลงตราประทับของบริษัทไว้ในเอกสารด้วย
การใดที่ผู้รับมอบอำนาจได้กระทำไปภายในขอบเขตตามหนังสือมอบอำนาจฉบับนี้ ย่อมมีผลผูกพันผู้มอบอำนาจตามกฎหมายทุกประการ เพื่อเป็นหลักฐานจึงได้ลงลายมือชื่อและประทับตราของผู้มอบอำนาจไว้เป็นสำคัญ ณ วันเดือนปีที่ระบุไว้ข้างต้น
