ต่อมา แม่น้ำเกิดการตื้นเขิน เพราะเปลี่ยนทิศทางเมื่อ ๑๐๐ ปีมาแล้ว และได้เกิดโรคอหิวาตกโรค (โรคห่า) ระบาดหนัก ชาวบ้านจึงได้อพยพไปอยู่ที่อื่น บ้านโพธิ์ประทับช้าง และ วัด จึงกลายเป็นหมู่บ้านร้าง ! โดยชาวบ้านส่วนใหญ่ได้ไปตั้งบ้านเรือนอยู่ริมฝั่ง แม่น้ำยม ภายหลังเมื่อมีผู้คนเข้ามาตั้งถิ่นฐานประกอบอาชีพกันหนาแน่นทางราชการเห็นว่า ตำบลโพธิ์ประทับช้าง และตำบลใกล้เคียงซึ่งขึ้นกับ อำเภอเมืองพิจิตร และอำเภอโพทะเล บางส่วนอยู่ไกลเกินไปไม่สะดวกในการติดต่อราชการ

จึงได้ยกฐานะเป็น กิ่งอำเภอโพธิ์ประทับช้าง เมื่อวันที่ ๒๔ มิถุนายน พ.ศ. ๒๕๑๐ และยกฐานะเป็นอำเภอโพธิ์ประทับช้างเมื่อวันที่ ๑ กันยายน ๒๕๑๖

ปัจจุบันที่ว่าการอำเภอโพธิ์ประทับช้าง ตั้งอยู่ที่ บ้านท่าตะค้อหมู่ที่ ๕ ตำบลโพธิ์ประทับช้าง ริมฝั่ง แม่น้ำยม ด้านตะวันตกเฉียงใต้ ของจังหวัด ห่างจากตัวจังหวัดไปทางทิศใต้ประมาณ ๒๔ กิโลเมตร ห่างจากกรุงเทพฯ ประมาณ ๓๓๔ กิโลเมตร มีพื้นที่การปกครองทั้งหมด ๕๗๘.๕๖ ตารางกิโลเมตร
