1. เอกสารประกอบการออก Form หลัก ๆ แล้วจะประกอบไปด้วย Invoice, ผลการตรวจคุณสมบัติทางด้านถิ่นกำเนิด, เอกสารทางด้านการขนส่ง ได้แก่ ทางเครื่องบินใช้ Air Waybill, ทางเรือใช้ Bill of Lading, ทางบก (รถบรรทุก) ใช้ Truck Receipt เท่านั้น ซึ่งการที่จะนำใบขนส่งสินค้าขาออกมาเป็นเอกสารประกอบการออก Form แทน Truck Receipt นั้นไม่สามารถกระทำได้
2. ผู้ประกอบการสามารถยื่นขอหนังสือรับรองถิ่นกำเนิดสินค้าประเภท Form D ย้อนหลัง หลังจากวันที่ส่งออก (Date of On-board)ได้ภายใน 1 ปี
3. ปัจจุบันสำนักบริการการค้าต่างประเทศ กรมการค้าต่างประเทศ ได้เปิดให้บริการขอหนังสือรับรองฯ ผ่่านระบบลายมือชื่ออิเล็กทรอนิกส์ (Digital Signature) ซึ่งเป็นระบบที่ผู้ประกอบการสามารถแนบ File เอกสารประกอบการออก Form มาพร้อมกับคำขอในระบบได้ แต่ผู้ประกอบการต้องลงทะเบียนขอเปิดใช้งานกับสำนักบริการการค้าต่างประเทศ กรมการค้าต่างประเทศ ก่อนการใช้งาน ศึกษารายละเอียดเพิ่มเติมได้ที่ www.dft.go.th>บริการข้อมูล> เอกสารเผยแพร่> ขั้นตอนการให้บริการ> ดาวน์โหลดไฟล์ "แผนผังแสดงขั้นตอนการสมัครใช้งานระบบการให้บริการออกหนังสือสำคัญฯด้วยลายมือชื่ออิเล็กทรอนิกส์ (Digital Signature)
4. หากส่งหลักฐานต่าง ๆ ทาง Online แล้วในวันรับ Form D ให้ยื่นเฉพาะบัตรประจำตัวผู้ประกอบการหรือบัตรประจำตัวผู้รับมอบอำนาจที่ใช้ Login เข้าระบบขอ Form ชุดนั้น โดยไม่ต้องนำเอกสารฉบับจริงมาแสดง
