*รัชกาลที่ 4 จึงโปรดเกล้าฯให้จัดตั้งเป็นเมือง โดยตั้งชื่อตามลำน้ำว่า “เมืองเลย”*

ในสมัยรัชกาลที่ 5 ได้มีการจัดการปกครองแบบใหม่ เรียกว่า “การปกครองแบบเทศาภิบาล” แบ่งเป็นมณฑลเมือง ตำบล หมู่บ้าน

เมืองเลยจึงเป็นเมืองหนึ่งซึ่งอยู่ในเขตปกครองของมณฑลลาวพวน ซึ่งต่อมาได้เปลี่ยนชื่อเป็น “มณฑลอุดร”

ครั้งแรกมี 3 อำเภอ คือ อำเภอท่าลี่ อำเภอนากอก และอำเภอกุดป่อง ซึ่งเป็นที่ตั้งของตัวเมือง (อำเภอเมืองเลยในปัจจุบัน)

ต่อมาวันที่ 4 มกราคม 2450 ให้ยกเลิกชื่อ “บริเวณลำน้ำเหือง” ให้คงชื่อไว้เฉพาะคำว่า “เมืองเลย” ขณะเดียวกันก็ให้เปลี่ยนชื่อ “อำเภอกุดป่อง” เป็น “อำเภอเมืองเลย”
