ความเป็นมาและความสำคัญของปัญหา
ปัจจุบันเราไม่อาจปฏิเสธได้ว่า สารเสพติด ได้เข้ามามีบทบาทกับชีวิตของเรามากขึ้น อาจเนื่องด้วยบุคคลในสังคม หรือคนในครอบครัว เข้าไปยุ่งเกี่ยวกับสารเสพติด ไม่ว่าจะเป็น สารเสพติดที่ผิดกฎหมายอย่างเช่น เมทแอมเฟตามีน โคเคน ไอซี และสารอื่นอีกมากมาย นอกจากสารเสพติดที่ผิดกฎหมายแล้วในบุคคลที่ปฏิเสธสารเสพติดเหล่านี้อาจเข้าไปเสพติด สารเสพติดในกลุ่มที่ถูกกฎหมาย อันได้แก่ บุหรี่ หรือ กลุ่มเครื่องดื่มแอลกอฮอล์ ซึ่งคนที่ดื่มแอลกอฮอล์หรือดูดบุหรี่ ได้ถูกมองเป็นเรื่องธรรมดา ที่เกิดขึ้นในสังคมทุกๆสังคมทั่วโลกในปัจจุบัน ซึ่งผลจากการเสพติดสารเสพติดกลุ่มที่ผิดกฎหมายนั้น นอกจากจะก่อให้เกิดผลเสียให้กับร่างกายของผู้เสพแล้ว ยังส่งผลกระทบต่อสังคมและประเทศชาติ ในด้านการเกิดอาชญากรรมในสังคม รวมถึงการสูญเสียงบประมาณในการนำมาบำบัดคนไข้ที่ติดสารเสพติดอีกด้วย
เมท แอมเฟตามีน (Methamphetamine) เป็นอนุพันธ์ที่อยู่ในกลุ่ม Amphetamine โดยสารกลุ่มนี้เป็นสารที่เป็นส่วนประกอบหลักในการผลิตยาบ้า(1) โดยวัตถุออกฤทธิ์ชนิดนี้มีฤทธิ์ทำลายระบบประสาท โดยการกระตุ้นประสาทบริเวณระบบประสาทส่วนกลาง ทำให้ผู้ที่เสพรู้สึกตื่นตัว กระปรี้กระเปร่า มีกำลัง ดังนั้น สารเสพติดกลุ่มนี้จึงถูกนำมาใช้ในการใช้เป็นยาเพิ่มกำลัง โดยที่บางคนอาจไม่ทราบถึงผลเสียที่ตามมา โดยการเสพติดยาเสพติด นั้นเกิดจาก เมื่อผู้เสพยา Methamphetamine แล้ว Methamphetamine จะออกฤทธิ์กระตุ้นการผลิตและส่งสัญญาณไฟฟ้าของเซลประสาทของสมองส่วนควบคุมความพอใจบริเวณ Brain reward pathway ให้ส่งสัญญาณไฟฟ้าไปตาม Axon(แขนส่งสัญญาณ) เป็น Pre-synaptic เมื่อมาถึง Synapse(กระเปาะปมประสาทบริเวณปลายแขนส่งสัญญาณ) จะกระตุ้นให้มีการหลั่ง Neurotransmitter : Dopamine ออกมามาก ที่ synaptic cleft แล้วจับกับ Dopamine receptor จาก Dendrite( แขนรับสัญญาณ) ของอีกเซลประสาทเป็น Post-synaptic

การเสพสารเสพติดกลุ่ม แอมเฟตามีนเป็นระยะเวลานานจนทำให้เสพติดได้ส่งผลกระทบต่อระบบประสาทได้ ซึ่งได้มีการศึกษาอย่างหลากหลาย เช่น การทดลองในเซลล์ทดลอง

จากอดีตถึงปัจจุบันได้มีการศึกษาผลกระทบของ แอมเฟตามีนต่อระบบสมอง ทั้งใน Cell cuture , สัตว์ทดลอง แต่ยังขาดการศึกษารายละเอียดเกี่ยวกับผลกระทบต่อระบบสมองในผู้ป่วยที่ติดสารเสพติดโดยตรง เนื่องจากการศึกษาที่ทำการศึกษาในมนุษย์โดยตรงยังมีข้อจำกัดในด้านจริยธรรม จึงเห็นควรมีการศึกษาคัดกรองเบื้องต้น โดยการใช้แบบคัดกรองเบื้องต้น เพื่อดูผลกระทบของแอมเฟตามีนต่อระบบสมองของมนุษย์ ซึ่งในต่างประเทศได้มีการใช้แบบคัดกรองหลายชนิดได้แก่ MCI, MOCA, MMSE, LOTCA, DSM-V เพื่อความแม่นยำ และความน่าเชื่อถือ และแบบคัดกรองที่ได้รับการยอมรับและเป็นที่นิยม จึงเลือกใช้แบบคัดกรอง 3 แบบด้วยกันได้แก่ LOTCA, MOCA, MMSE
โดยในต่างประเทศได้เครื่องมือเหล่านี้ในการศึกษาในคนไข้สมองเสื่อมจากโรคพากินส์สัน(6) แต่ยังไม่มีการใช้แบบคัดกรองที่มากถึง 3 แบบ เพื่อคัดกรองศึกษาผลของแอมเฟตามีนต่อสมองว่ามีผลกระทบใดบ้าง จึงเป็นที่น่าสนใจที่ควรทำการศึกษาต่อไป
วัตถุประสงค์ของการศึกษา
1.เพื่อศึกษาผลกระทบของสารแอมเฟตามีนต่อระบบประสาทของผู้ที่ติดสารเสพติดกลุ่ม แอมเฟตามีน
2.เพื่อทบทวนแนวคิด และศึกษาการเกิดโรคสมองเสื่อมในผู้ที่ติดสารเสพติด
ขอบเขตของการศึกษา
การศึกษาครั้งนี้เป็นการวิจัยเชิงสำรวจ เพื่อศึกษาผลกระทบจากสารแอมเฟตามีนต่อการเกิดสมองเสื่อมของคนไข้ที่ติดสารเสพติดกลุ่มแอมเฟตามีน เทียบกับ คนปกติ โดยกลุ่มตัวอย่างที่ติดสารเสพติดเป็นคนไข้ที่ รพ ธัญญารักษ์ ปทุมธานี และตัวอย่างกลุ่มคนปกติ เป็นนักศึกษา มหาวิทยาลัยมหิดล โดย ใช้ตัวอย่าง 200 คน แบ่งกลุ่มละ 100 โดยทำการเก็บข้อมูลใน
