ประวัติความเป็นมาเมืองเชียงคานเดิมตั้งอยู่ที่เมืองชะนะคาม ประเทศลาว ซึ่งสร้างโดยขุนคาน โอรสของขุนคัวแห่งอาณาจักรล้านช้าง เมื่อประมาณ พ.ศ. 1400 ต่อมาประมาณ พ.ศ. 2250 อาณาจักรล้านช้างแยกออกเป็นสองอาณาจักรคือ อาณาจักรหลวงพระบาง ซึ่งมีพระเจ้ากีสราชเป็นกษัตริย์ และอาณาจักรเวียงจันทน์ ซึ่งมีพระเจ้าไชยองค์เว้เป็นกษัตริย์ โดยกำหนดอาณาเขตให้ดินแดนเหนือแม่น้ำเหืองขึ้นไปเป็นอาณาเขตหลวงพระบาง และใต้แม่น้ำเหืองลงมาเป็นอาณาเขตเวียงจันทน์ ต่อมาทางหลวงพระบางได้สร้างเมืองปากเหืองซึ่งอยู่ฝั่งขวาของแม่น้ำโขงเป็นเมืองหน้าด่านและทางเวียงจันทน์ได้ตั้งเมืองเชียงคาน
