ตั้งอยู่ในบ้านนาเหนือ ตำบลปากแพรก อำเภอทุ่งสง ประชาชนชาวอำเภอทุ่งสงและใกล้เคียงศรัทธาและกราบไหว้บูชามากกว่า 60 ปี นอกจากจะเป็นที่ประดาฐานองค์พระเจ้าซำปอกง(หรือหลวงพ่อโต)แล้วยังมีพระโพธิสัตว์กวนอิม องค์ใหญ่ที่สุดในประเทศไทยแล้ว มีพระโพธิสัตว์ศรีอาริยเมตไตรย(พระสังกัจจายน์) องค์ฮูเทียนเซียงตี่ องค์กวงเทียนฮุคโจ้ว องค์เล่าอวบซือกง องค์กิมบ่อเนี้ย องค์เทวะโป้ยเซียน เละพระภูมิเจ้าที่(แป๊ะกง)
มีบันทึกบอกเล่าประวัติว่า เมื่อ พ.ศ. 2495 มีคณะบุคคลที่เลื่อมใสและศรัทธาในพระซำปอกงหรือหลวงพ่อโต ได้อัญเชิญกระถางธูปของพระซำปอกงมาตั้งเพื่อใช้สักการบูชา ณ บริเวณบ้านนาเหนือ ตำบลปากแพรก อำเภอทุ่งสง ต่อ พ.ศ.2514 ได้สร้างวิหารหลังใหม่ขึ้นพร้อมกับสร้างพระพุทธรูปซำปอกงองค์ใหม่ เป็นปูนปั้นขนาดหน้าตัดกว้าง 52 นิ้ว ประดิษฐานเป็นพระประธาน ในวันที่ 31 สิงหาคม พ.ศ.2538 ได้รับอนุญาตให้จดทะเบียนจัดตั้งเป็นมูลนิธิ โดยมีวัตถุประสงค์เพื่อทำนุบำรุงพระพุทธศาสนา การศึกษา วัฒนธรรม และช่วยเหลือ และผู้ประสบภัยพิบัติต่างๆ
