เนื่องจากว่าที่ชื่อยางไม่ได้ระบุว่า Lenso Co., Ltd. ค่ะ

แนบตัวอย่าง Form D ให้ดูมา 3 แบบนะคะ ที่จะแก้ไข shipment ล่าสุดนี้ค่ะ

1.กรณีแก้ไข Form D
ในกรณีที่เราส่ง Form D ไปให้ MSA แก้ไข คือต้องขีดฆ่า คาว่า Lenso Co., Ltd. ทิ้ง
และใส่คาว่า N/M แทนค่ะ โดยประทับ+พร้อมเซ็นชื่อกากับด้วยค่ะ

2. กรณีขอ Form D ใหม่
ในกรณีที่ MSA จะขอ Form D ให้เราใหม่ คือต้องพิมคาว่า N/M ในช่องหมายเลข 6
และให้ Remark: ว่าอ้างถึง Form D เลขที่อะไรและลงวันที่เท่าไหร่ตามเอกสารแนบค่ะ
