ผนังกั้นช่องจมูกคด...ผ่าตัดแก้ไขอย่างไร (Septoplasty)

การที่มีผนังกั้นช่องจมูกคด (deviated nasal septum)  จะทำให้มีอาการคัดจมูกข้างใดข้างหนึ่งมากกว่าอีกข้างหนึ่งได้  นอกจากนั้นอาจเป็นสาเหตุของการปวดในโพรงจมูก (sinonasal pain),  ไซนัสอักเสบเฉียบพลันที่กลับเป็นซ้ำ (recurrent acute sinusitis), ไซนัสอักเสบเรื้อรัง (chronic sinusitis), โรคริดสีดวงจมูก (nasal polyp), โรคของหูชั้นกลาง [เช่นการอุดตันของท่อยูสเตเชียน (eustachian tube dysfunction)ทำให้มีหูอื้อ หรือ เสียงดังในหู], เลือดกำเดาไหล (epistaxis)  และอาการนอนกรน (snoring) และ/ หรือมีภาวะหยุดหายใจขณะหลับ (obstructive sleep apnea) 

การผ่าตัดแก้ไขภาวะดังกล่าว  ทำโดยการผ่าตัดผ่านช่องจมูก  โดยจะไม่มีแผลหรือรอยผ่าตัดให้เห็นภายนอก  ยกเว้นในรายที่ผนังกั้นช่องจมูกคดนั้นอยู่บริเวณด้านหน้ามาก อาจมีแผลเล็กๆที่ผิวหนังบริเวณผนังกั้นช่องจมูกทางด้านหน้าส่วนล่าง ซึ่งเล็กและแทบมองไม่เห็น แพทย์จะเย็บแผลผ่าตัดในโพรงจมูกด้วยไหมละลาย  การผ่าตัดนี้บางครั้งจะทำร่วมกับ การใช้คลื่นความถี่วิทยุลดขนาดของเยื่อบุจมูก, การผ่าตัดรักษาโรคไซนัสอักเสบ หรือโรคริดสีดวงจมูกโดยวิธีการส่องกล้อง  

การผ่าตัดนี้อาจใช้ยาชาเฉพาะที่ หรือวิธีดมยาสลบ ทั้งนี้ขึ้นอยู่กับแพทย์ผู้ทำการผ่าตัด และผู้ป่วย

    

ก่อนผ่าตัด  แพทย์จะตรวจความสมบูรณ์ของร่างกาย และประเมินความพร้อมของผู้ป่วยก่อนผ่าตัด เช่นการตรวจเลือด  ปัสสาวะ  ถ่ายภาพรังสีปอด หรือ ตรวจคลื่นไฟฟ้าหัวใจ แล้วแต่ความจำเป็นในผู้ป่วยแต่ละราย  สำหรับผู้ป่วยที่รับประทานยาบางชนิด เช่น ยาขยายหลอดเลือด, ยาแอสไพริน หรือ ยาต้านการแข็งตัวของเลือด  แพทย์อาจให้หยุดยาดังกล่าวก่อนผ่าตัดหลายวัน  ผู้ป่วยควรรักษาสุขภาพให้แข็งแรง เช่น พักผ่อนอย่างเพียงพอ  หลีกเลี่ยงคนรอบข้างที่ไม่สบาย เพื่อป้องกันไข้หวัด หรือการติดเชื้อของระบบทางเดินหายใจ ซึ่งอาจทำให้ต้องเลื่อนการผ่าตัด 

 

การใช้ยาชาเฉพาะที่  ถ้าแพทย์ทำผ่าตัดโดยใช้ยาชาเฉพาะที่  ผู้ป่วยสามารถมาโรงพยาบาลวันที่นัดทำผ่าตัดได้เลย  การใช้ยาชาเฉพาะที่ มีโอกาสเสี่ยงที่จะเกิดภาวะแทรกซ้อน เช่น ใจสั่น หน้ามืด  เป็นลม  หูอื้อ แต่อาการเหล่านี้มักหายได้เอง  

การดมยาสลบ  ผู้ป่วยจะต้องเข้ามาอยู่ในโรงพยาบาล 1 วันก่อนผ่าตัด เพื่อวิสัญญีแพทย์จะได้เตรียมความพร้อมสำหรับการดมยาสลบในวันรุ่งขึ้นที่จะผ่าตัด  คืนวันก่อนผ่าตัด แพทย์จะให้งดน้ำและอาหารหลังเที่ยงคืนเพื่อป้องกันการสำลักอาหาร และน้ำลงปอดเวลาดมยาสลบ  การดมยาสลบ  มีโอกาสเสี่ยงที่จะเกิดภาวะแทรกซ้อน เช่น เสียงแหบจากสายเสียงบวม  ปอดอักเสบจากการสูดสำลัก หายใจลำบาก เนื่องจากทางเดินหายใจบวม แต่ภาวะแทรกซ้อนเหล่านี้พบได้น้อยมาก  ในรายที่มีอาการดังกล่าวรุนแรงมาก  อาจต้องใส่ท่อช่วยหายใจ (endotracheal tube) ไว้ หลังผ่าตัดเสร็จ  และนอนในหอผู้ป่วยหลังผ่าตัด  1 คืน เพื่อสังเกตการหายใจหลังผ่าตัด

คืนวันก่อนผ่าตัด  ผู้ป่วยจะได้รับการตัดขนจมูก  เพื่อเตรียมบริเวณที่จะทำการผ่าตัด  

 

หลังผ่าตัด  จะมีวัสดุที่ใช้ในการห้ามเลือดอยู่ในโพรงจมูกทั้งสองข้าง  ทำให้ผู้ป่วยมีอาการคัดจมูก จึงต้องหายใจทางปากจนกว่าแพทย์จะเอาวัสดุห้ามเลือดออก ซึ่งอาจทำให้คอแห้ง แสบหรือเจ็บคอได้ จึงควรจิบน้ำ และกลั้วคอบ้วนปาก เพื่อทำความสะอาดช่องปากและคอบ่อยๆ        

ผู้ป่วยอาจมีอาการเจ็บจมูกจากแผลผ่าตัดบ้าง  หรือมีเลือดปนน้ำมูกหรือน้ำลายได้บ้าง ผู้ป่วยไม่ควรดึงวัสดุห้ามเลือดในโพรงจมูกออกเอง เพราะอาจทำให้มีเลือดออกมากได้  หลังผ่าตัดผู้ป่วยจะได้รับยาต้านจุลชีพ, ยาแก้ปวด, ยาลดบวม, ยาหดหลอดเลือดชนิดหยอดจมูกเพื่อห้ามเลือด (topical decongestant), ยาลดอาการคัน จาม คัดจมูก น้ำมูกไหล  และจะมีสายให้น้ำเกลืออยู่ที่แขน  เมื่อผู้ป่วยรับประทานได้ดีพอควร  แพทย์จะเอาสายให้น้ำเกลือออก  ผู้ป่วยสามารถรับประทานยาแก้ปวด เช่น พาราเซตามอล เมื่อจำเป็นได้ 

แพทย์จะนำวัสดุที่ใส่ไว้ในโพรงจมูกออกภายใน 24-48 ชั่วโมง หลังผ่าตัด พยาบาลจะสอนให้ล้างทำความสะอาดช่องจมูกด้วยตนเอง เพื่อทำการล้างจมูกต่อที่บ้าน ซึ่งโดยทั่วไป ผู้ป่วยควรล้างจมูก หลังจากที่แพทย์เอาวัสดุห้ามเลือดออกแล้ว ประมาณ48 ชั่วโมง ถ้าไม่มีเลือดออกที่ผิดปกติ  ผู้ป่วยควรล้างจมูกทำความสะอาดโพรงจมูกด้วยน้ำเกลืออุ่น วันละ 2 – 4 ครั้ง ในช่วงหลังเอาวัสดุห้ามเลือดออกใหม่ ๆ  อาจจะมีไข้ หรือมีอาการบวม หรือรู้สึกติดๆ ขัดๆ ตึงๆคล้ายมีสิ่งแปลกปลอมอยู่ในโพรงจมูกหรือมีเสียงเปลี่ยนได้  ซึ่งอาการดังกล่าวมักจะหายไปเองภายใน 1 สัปดาห์  หลังจากที่แพทย์เอาวัสดุห้ามเลือดออกแล้ว 24-48 ชั่วโมง เยื่อบุจมูกอาจบวมมากขึ้น ทำให้อาการคัดจมูกมากขึ้นได้  ดังนั้นจึงควรนอนศีรษะสูง โดยใช้หมอนหนุน หรือนอนบนที่นอนที่สามารถปรับความเอียงได้  อมและประคบน้ำแข็งบ่อยๆบริเวณหน้าผากหรือลำคอ ในช่วงสัปดาห์แรก  เพื่อลดอาการบวมและเลือดออกบริเวณที่ทำผ่าตัด           

            โดยทั่วไป ถ้าไม่มีภาวะแทรกซ้อน ผู้ป่วยจะกลับบ้านได้ หลังผ่าตัด 3-4 วัน แพทย์จะให้ผู้ป่วยพักรักษาตัวที่บ้านประมาณ 1 สัปดาห์   ผู้ป่วยสามารถทำกิจวัตรประจำวันได้ตามปกติ  แต่ควรหลีก
เลี่ยงการสั่งน้ำมูกแรงๆ   การแคะจมูก หรือการกระทบกระเทือนบริเวณจมูก  การออกแรงมาก   การเล่นกีฬาที่หักโหม หรือยกของหนักหลังผ่าตัด  เพราะอาจทำให้มีเลือดออกจากแผลที่เยื่อบุจมูกได้  ถ้ามีเลือด
ออกจากจมูกหรือไหลลงคอ ควรนอนพัก ยกศีรษะสูง  หยอดยาหดหลอดเลือดชนิดหยอดจมูกเพื่อห้ามเลือดที่แพทย์สั่งไว้ให้ 3-4 หยดในโพรงจมูกแต่ละข้าง  นำน้ำแข็งมาประคบบริเวณหน้าผากหรือคอ  อมน้ำแข็งเพื่อให้เลือดหยุด  การประคบ หรืออมน้ำแข็ง  ควรประคบหรืออมประมาณ 10 นาที แล้วจึงเอาออกประมาณ 10 นาที  แล้วค่อยประคบหรืออมใหม่เป็นเวลา 10 นาที  ทำเช่นนี้สลับกันไปเรื่อยๆ    ถ้าเลือดออกไม่หยุด หรือออกมากผิดปกติ เช่น เป็นถ้วยแก้ว ควรรีบมาโรงพยาบาลทันที
