Contrary to expectation, neck pain and stiffness are generally not the most common reports with SCM TrPs, although this muscle may add an additional component to the “stiff neck” syndrome, which is primarily due to TrP activity in the levator scapulae, posterior cervical, and trapezius muscles. Trigger points in this muscle may cause tilting of the head to the ipsilateral side, because holding the head upright causes pain.
