สัญญาว่าจ้างนักกีฬาฉบับนี้ ทำขึ้นที่ สโมสร ฟุตบอลอุทัยธานี ฟอเรสท์ . .ตั้งอยู่ที่ สนามกีฬาจังหวัดอุทัยธานี ถนนรักการดี ตำบลอุทัยใหม่ อำเภอเมืองอุทัยธานี จังหวัดอุทัยธานี 6100 เมื่อวันที่ 15 เดือน มกราคม พ.ศ. 2557 ระหว่างสโมสร ฟุตบอลอุทัยธานี ฟอเรสท์ . โดยแผนกฟุตบอลทำการแทนโดย นายเผด็จ นุ้ยปรี ซึ่งต่อไปนี้ในสัญญานี้ เรียกว่า “สโมสร” ฝ่ายหนึ่งกับ นายบัญชา อินโต อายุ 23 ปีอยู่บ้านเลขที่ . . .บ้านพักสนามกีฬาจังหวัดอุทัยธานี เลขบัตรประจำตัวประชาชน/.11799 00199 186. ออกให้โดย กระทรวงมหาดไทย วันออกบัตร 3 พ ค 52 วันหมดอายุ 8 มี.ค.59 . ซึ่งต่อไปในสัญญานี้เรียกว่า “นักกีฬา” อีกฝ่ายหนึ่ง โดยมีข้อสัญญาดังต่อไปนี้
ข้อ 1. สโมสรตกลงว่าจ้าง “นักกีฬา” เป็นนักกีฬาของสโมสรมีกำหนด 1 ปี นับตั้งแต่ วันที่ 15 เดือน มกราคม พ.ศ. 2557 สิ้นสุดวันที่ 30 เดือน กันยายน พ.ศ. 2557 .และก่อนสิ้นสุดสัญญา คู่สัญญาอาจต่อสัญญาออกไปได้อีก ตามที่จะได้ตกลงกันเป็นคราวๆไป
ข้อ 2. สโมสร ตกลงจะจ่ายค่าจ้างให้นักกีฬา ดังนี้
2.1 ค่าจ้างเป็นรายเดือนๆ ละ 20,000.- บาท ( สองหมื่นบาทถ้วน ) หากทำการฝึกซ้อมครบตามวันซ้อมที่กำหนดไว้ โดยกำหนดจ่ายค่าจ้างทุกวันที่ 5 ของเดือนถัดไป
2.2 ค่าเบี้ยฝึกซ้อมตามที่ไปฝึกซ้อม วัน - บาท ( - ) โดยกำหนดจ่ายทุกวันที่ 15 ขอเดือนถัดไป
2.3 ผลประโยชน์ตอบแทนอื่นๆ ให้เป็นไปตามระเบียบของสโมสร หรือตามมติที่ประชุมของคณะกรรมการแผนกฟุตบอลที่กำหนดไว้ หรือที่จะกำหนดขึ้นมาใหม่ในภายหน้า
ข้อ 3. นักกีฬาขอรับรองว่า
3.1 จะปฏิบัติตามระเบียบของสโมสรที่กำหนดไว้ในขณะทำสัญญานี้ หรือที่จะกำหนดขึ้นมาใหม่ในภายหน้าทุกประการ
3.2 จะใช้ความสามารถเต็มที่ในการฝึกซ้อม – แข่งขันและปฏิบัติตามนโยบายของผู้ฝึกสอน หรือเจ้าหน้าที่ทีม หรือผู้บริหารทีมอย่างเคร่งครัด
3.3 จะตั้งใจแข่งขันเพื่อชัยชนะ และชื่อเสียงของสโมสร
3.4 จะไม่ไปเล่นฟุตบอลให้กับทีมฟุตบอลอื่น หรือไปมีส่วนร่วมในการโฆษณาประชาสัมพันธ์ให้กับธุรกิจใดๆ เว้นแต่จะได้รับอนุญาตเป็นลายลักษณ์อักษรจากสโมสรแล้ว


ข้อ 4. สโมสร...

ข้อ 4. สโมสร มีสิทธิบอกเลิกสัญญาได้ทันที และไม่ต้องจ่ายเงินหรือค่าตอบแทนใดๆ ให้กับนักกีฬาในกรณีต่อไปนี้
4.1 ถึงแก่กรรม หรือสิ้นสุดสัญญา
4.2 ลาออก และได้รับอนุญาตจากสโมสรแล้ว
4.3 ประพฤติผิดระเบียบวินัย หรือข้อบังคับ หรือทำให้เกิดความเสื่อมเสียชื่อเสียงแก่สโมสร หรือกระทำการใดๆอันผิดปกติวิสัยของนักกีฬาที่ดี
4.4 ก่อการทะเลาะวิวาท หรือยุแหย่ ให้เกิดความแตกแยก หรือสร้างความขัดแย้งให้เกิดขึ้นในหมู่นักกีฬา
4.5 ไม่ให้เกียรติแก่ผู้ฝึกสอน หรือเจ้าหน้าที่ทีม หรือผู้บริหารทีม หรือผู้บริหารของสโมสร
ข้อ 5. การย้าย หรือการลาออกสโมสร นักกีฬาจะต้องจัดทำเป็นลายลักษณ์อักษร โดยต้องปฏิบัติตามสัญญา และต้องได้รับอนุมัติจากสโมสรก่อน โดยสโมสรมีสิทธิเรียกเงินชดเชยการโอนย้าย หรือการลาออกจากนักกีฬาได้
ข้อ 6. หากนักกีฬาปฏิบัติผิดสัญญาข้อใดข้อหนึ่งในสัญญานี้ นักกีฬายินยอมชดใช้เบี้ยปรับให้แก่สโมสรเป็นจำนวนเงิน 30,000 บาท ( สามหมื่นบาทถ้วน) และหากมีความเสียหายเกิดขึ้นแก่สโมสร นักกีฬายินยอมชดใช้ค่าเสียหายที่เกิดขึ้นแก่สโมสร อีกต่างหากด้วย
ข้อ 7. ทั้งสองฝ่าย ได้เข้าใจในสัญญาฉบับนี้ตรงกันว่า สโมสรมีฐานะเป็น “ผู้ว่าจ้าง” (ผู้จ้างทำงาน) “นักกีฬา” เป็นผู้รับจ้าง (รับจ้างทำงาน) มีความผู้พันในความสำเร็จทางการฝึกซ้อม – แข่งขันกีฬาตามนโยบายของสโมสรเท่านั้น และตกลงถือปฏิบัติตามระเบียบของสโมสรที่มีอยู่แล้วในขณะนี้ หรือที่จะมีขึ้นภายหน้า หากนักกีฬาผิดสัญญาข้อใดข้อหนึ่ง สโมสรมีสิทธิบอกเลิกสัญญาได้ทันที โดยไม่ต้องจ่ายเงิน หรือค่าตอบแทนใดๆ ให้กับนักกีฬา
สัญญานี้ ทำขึ้นเป็น 4 ฉบับมีข้อความถูกต้องตรงกัน ทั้ง 2 ฝ่าย ได้อ่านเข้าใจโดยตลอด จึงได้ลงลายมือชื่อต่อหน้าพยาน ไว้เป็นหลักฐาน
