อุทยานแห่งชาติเขาเเหลมหญ้า-หมู่เกาะเสม็ด ตั้งอยู่บริเวณชายฝั่งทะเลด้านอ่าวไทยในท้องที่อำเภอแกลง และอำเภอเมือง จังหวดระยอง ครอบคลุมพื้นที่บนฝั่งและในท้องทะเล ตลอดจนเกาะต่างๆ ประกอบด้วยเกาะเสม็ด เกาะจันทร์ เกาะทะลุ เกาะกุฎี เกาะมะขาม และเกาะปลายตีน เขาแหลมหญ้าและชายทะเลด้านทิศตะวันตกของเขาแหลมหญ้า สถานที่ที่เด่นที่สุดเป็นที่รู้จักกันดี คือ เกาะเสม็ดหรือเกาะแก้วพิศดาร ซึ่งกล่าวไว้ในวรรณคดีเรื่องพระอภัยมณี ความงดงามตามธรรมชาติของอุทยานแห่งชาติแห่งนี้เป็นที่ยอมรับของนักท่องเที่ยวทั้งชาวไทยและชาวต่างประเทศว่า มีทิวทัศน์สวยงาม เหมาะสำหรับการพักผ่อน

ในปี พ.ศ. 2518 ได้มีนายทุนกลุ่มหนึ่ง รวมหุ้นกันเพื่อดำเนินการขอเช่าเกาะเสม็ดจากจังหวัดระยอง เพื่อจัดเป็นแหล่งท่องเที่ยว โดยจะสร้างกระเช้าลอยฟ้าข้ามจากฝั่งเขาแหลมหญ้าไปยังเกาะเสม็ดเพื่อบริการนักท่องเที่ยว แต่โครงการต้องระงับไปเพราะได้ถูกกลุ่มนิสิต นักศึกษาเป็นจำนวนมากจากหลายสถาบันเดินขบวนคัดค้าน และในปีเดียวกันนั้นสมาคมอนุรักษ์ศิลปกรรมและสิ่งแวดล้อม ได้มีหนังสือที่ 538/2518 ลงวันที่ 24 ธันวาคม 2518 ถึงผู้ว่าราชการจังหวัดระยอง ให้ความเห็นว่า ควรมีการอนุรักษ์เกาะเสม็ด (เกาะแก้วพิสดารในเรื่องพระอภัยมณีอันเป็นวรรณกรรมเอกของสุนทรภู่) ซึ่งมีความสำคัญทางประวัติศาสตร์ สงวนไว้เป็นสมบัติของประชาชนโดยทั่วไป โดยจัดตั้งเป็นวนอุทยานทางทะเล และคณะกรรมการอุทยานแห่งชาติได้มีมติในการประชุม ครั้งที่ 1/2519 เมื่อวันที่ 21 เมษายน 2519 ให้กรมป่าไม้ประสานกับจังหวัดระยอง พิจารณาในการจัดตั้งเกาะเสม็ดเป็นอุทยานแห่งชาติ ขณะนั้นเกาะเสม็ดอยู่ในความรับผิดชอบของกองทัพเรือ กรมป่าไม้จึงได้มีหนังสือ ที่ กษ 0808/1713 ลงวันที่ 31 มกราคม 2522 ขอพื้นที่ดังกล่าวไปอยู่ในความควบคุมดูแลของกรมป่าไม้ ซึ่งกองทัพเรือได้มีหนังสือ ที่ กห 0352/6405 ลงวันที่ 30 มีนาคม 2522 ตอบไม่ขัดข้องที่จะให้กรมป่าไม้เข้าควบคุมดูแลเกาะเสม็ด และต่อมาคณะกรรมการสิ่งแวดล้อมแห่งชาติได้มีมติในการประชุมครั้งที่ 5/2522 เมื่อวันที่ 19 ธันวาคม 2522 แจ้งให้กรมป่าไม้ดำเนินการอนุรักษ์เกาะเสม็ดเป็นอุทยานแห่งชาติทางทะเล

กองอุทยานแห่งชาติ กรมป่าไม้ จึงได้มีคำสั่ง ที่ 82/2523 ลงวันที่ 14 มกราคม 2523 ให้นายสมบูรณ์ วงศ์ภักดี ออกไปทำการสำรวจพื้นที่ป่าไม้บริเวณเขาแหลมหญ้าและบริเวณข้างเคียง ท้องที่บ้านเพ อำเภอเมือง จังหวัดระยอง เพื่อกำหนดเป็นอุทยานแห่งชาติ และให้ทำการจัดตั้งเป็นวนอุทยานเขาแหลมหญ้า ซึ่งจากการสำรวจของสมบูรณ์ วงศ์ภักดี พบว่า เขาแหลมหญ้าประกอบด้วยเขาดินลูกรังเตี้ยๆ ชายทะเลทางทิศตะวันตก มีหาดทรายกว้าง และทอดตัวโค้งยาวออกไปสวยงามมาก น้ำทะเลใสจนสามารถมองเห็นแนวปะการังที่สวยงามได้อย่างชัดเจน เกาะเสม็ดประกอบด้วยหาดทรายขาวรอบเกาะ เป็นหาดทรายที่สวยงาม เหมาะแก่การท่องเที่ยวพักผ่อน ทั้งมีเกาะเล็กเกาะน้อยที่มีหาดทรายและธรรมชาติสวยงาม เหมาะสมต่อการจัดตั้งเป็นอุทยานแห่งชาติ

กองอุทยานแห่งชาติ กรมป่าไม้ ได้นำผลการสำรวจเสนอคณะกรรมการอุทยานแห่งชาติ ซึ่งมีมติในการประชุมครั้งที่ 4/2523 เมื่อวันที่ 23 ธันวาคม 2523 เห็นสมควรให้ออกพระราชกฤษฎีกากำหนดบริเวณที่ดินป่าเขาแหลมเทียน เขาเปล็ด เขาแหลมหญ้า เกาะเสม็ด และเกาะใกล้เคียง ในท้องที่ตำบลเพ อำเภอเมืองระยอง และตำบลแกลง อำเภอแกลง จังหวัดระยอง เนื้อที่ประมาณ 81,875 ไร่ หรือ 131 ตารางกิโลเมตร ให้เป็นอุทยานแห่งชาติ โดยประกาศในราชกิจจานุเบกษา ฉบับพิเศษ หน้า 6 เล่ม 98 ตอนที่ 162 ลงวันที่ 1 ตุลาคม 2524 นับเป็นอุทยานแห่งชาติลำดับที่ 34 ของประเทศไทย
