ตั้งอยู่ที่บ้านเมืองเก่า หมู่ที่ 11 ตำบลบ้านยาง อำเภอเกษตรสมบูรณ์ จังหวัดชัยภูมิ
ในสมัยโบราณเดิม อำเภอเกษตรสมบูรณ์ จะเรียกว่า เมืองเก่าหรือ เมืองกลาง จะมีกองทัพและชนชาวลาวได้ยกทัพมาตั้งถิ่นฐานอยู่บริเวณหนองจอก (พระธาตุกุดจอกในปัจจุบัน ) เพราะเห็นว่ามีความอุดมสมบูรณ์เหมาะแก่การตั้งกองทัพ และถิ่นฐานต่อมาชาวบ้านได้เชื่อว่ามีงูทรวง หรือพญานาค ลงมาจากเขาเขียว และงูทรวงได้มาอาศัยอยู่บริเวณพระธาตุกุดจอก เมื่อก่อนบริเวณนี้เป็นลานกว้าง มีหญ้าขึ้นรอบ ๆ งูจะลงมาเล่นในหนองน้ำทุก ๆวันขึ้น 15 ค่ำ ตรงที่งูอาศัยอยู่จะมีลักษณะเป็นรอยแป่ว( แป่ว คือ รูที่มีลักษณะเป็นช่อง หรือ รูที่สัตว์ใช้สำหรับหายใจ,หลบภัย)
