สมเด็จพระญาณสังวร สมเด็จพระสังฆราช สกลมหาสังฆปริณายก มีพระนามเดิมว่า เจริญ คชวัตร ประสูติเมื่อวันที่ 3 ตุลาคม พ.ศ.2456 เป็นบุตรคนโตของนายน้อย คชวัตร และนางกิมน้อย คชวัตร ชาวกาญจนบุรี พระองค์มีน้องชาย 2 คน ได้แก่ นายจำเนียร คชวัตรและนายสมุทร คชวัตร บิดาของพระองค์ป่วยเป็นโรคเนื้องอกและเสียชีวิตไปตั้งแต่พระองค์ยังเล็ก

เมื่อพระองค์ยังทรงพระเยาว์นั้นทรงเจ็บป่วยออดแอดอยู่เสมอ โดยมีอยู่คราวหนึ่งที่ทรงป่วยหนักจนญาติ ๆ ต่างพากันคิดว่าคงไม่รอดแล้วและได้บนไว้ว่า ถ้าหายป่วยจะให้บวชเพื่อแก้บน แต่เมื่อหายป่วยแล้ว พระองค์ก็ยังไม่ได้บวช จนกระทั่งเรียนจบชั้นประถม 5 แล้วพระองค์จึงได้ทรงบรรพชาเป็นสามเณรเพื่อแก้บนในปี พ.ศ. 2469 ขณะมีพระชนมายุได้ 14 พรรษา ที่วัดเทวสังฆาราม โดยมีพระเทพมงคลรังษี (ดี พุทธฺโชติ) เจ้าอาวาสวัดเทวสังฆารามเป็นพระอุปัชฌาย์และพระครูนิวิฐสมาจาร (เหรียญ สุวณฺณโชติ) เจ้าอาวาสสวัดศรีอุปลารามเป็นพระอาจารย์ให้สรณะและศีล ภายหลังบรรพชาแล้วได้จำพรรษาอยู่ที่วัดเทวสังฆาราม 1 พรรษาและได้มาศึกษาพระธรรมวินัยที่วัดเสน่หา จังหวัดนครปฐม

กระทั่งชันษาครบอุปสมบท จึงได้เดินทางกลับไปอุปสมบทที่วัดเทวสังฆารามเมื่อพ.ศ. 2476 ภายหลังจึงได้เดินทางเข้ามาจำพรรษา ณ วัดบวรนิเวศวิหาร กรุงเทพมหานคร เพื่อทรงศึกษาพระธรรมวินัยต่อไป และที่วัดบวรนิเวศวิหารนี่เองพระองค์ท่านได้เข้าพิธีอุปสมบทซ้ำเป็นธรรมยุติกนิกาย โดยมีสมเด็จพระสังฆราชเจ้า กรมหลวงวชิรญาณวงศ์เป็นพระอุปัชฌาย์
