ธงชาติมาเลเซีย มีแถบสีแดงสลับสีขาวรวม 14 แถบ แต่ละแถบมีความกว้างเท่ากัน ที่มุมธงด้านคันธงมีรูปสี่เหลี่ยมผืนผ้าสีน้ำเงินกว้าง 8 ใน 14 ส่วนของผืนธงด้านกว้าง และยาวกึ่งหนึ่งของผืนธงด้านยาว ภายในรูปสี่เหลี่ยมผืนผ้าดังกล่าว มีเครื่องหมายพระจันทร์เสี้ยว และดาว 14 แฉก ซึ่งสี และสัญลักษณ์ ต่าง ๆ มี
ความหมาย ดังนี้

แถบริ้วสีแดง และสีขาว ทั้ง 14 ริ้ว หมายถึง สถานะอันเสมอภาคของรัฐสมาชิกทั้ง 13 รัฐ ภายในประเทศมาเลเซีย

ดาว 14 แฉก หมายถึง ความเป็นเอกภาพในหมู่รัฐดังกล่าวทั้งหมด

พระจันทร์เสี้ยว หมายถึง ศาสนาอิสลามอันเป็นศาสนาประจำชาติ

สีเหลืองในพระจันทร์เสี้ยว และดาว 14 แฉกคือสีแห่งยังดี เปอร์ตวน อากง ผู้เป็นประมุขแห่งสหพันธรัฐ 

สีน้ำเงิน หมายถึง ความสามัคคีของชาวมาเลเซีย
 
ธงชาติมาเลเซียแบบแรกสุดคือธงชาติสหพันธ์รัฐมาลายา ลักษณะเป็นธงสี่เหลี่ยมผืนผ้ามีแถบสี 4 สีขนาดเท่ากันเรียงจากบนลงล่าง คือ สีขาว สีแดง สีเหลือง และดำ ตรงกลางผืนธงมีตราเสือเผ่นบนพื้นวงกลมสีขาว เริ่มใช้ในปี พ.ศ. 2439 โดยธงนี้จะต้องชักคู่กับธงชาติสหราชอาณาจักรด้วย

ต่อมาในปี พ.ศ.2493 ได้มีการเปลี่ยนธงชาติสหพันธ์รัฐมาเลเซียมาเป็นธงริ้วแดงสลับขาว 11 แถบ ประกอบด้วยพระจันทร์เสี้ยวและดาราแห่งสหพันธ์ 11แฉกในพื้นสี่เหลี่ยมสีน้ำเงิน เพื่อแทนรัฐทั้ง 11 รัฐ คือ ปะลิส ไทรบุรี เประ กลันตัน ตรังกานู ปะหัง เนกรีเซมบิลัน สลังงอร์ ปีนัง ยะโฮร์ และมะละกา ธงนี้ได้ชักขึ้นเป็นครั้งแรกคู่กับธงชาติสหราชอาณาจักรเมื่อวันที่ 26 พฤษภาคม พ.ศ. 2493 ที่หน้าพระราชวังสลังงอร์ (Istana Selangor) และได้ชักขึ้นเป็นธงรัฐเอกราชครั้งแรกที่หน้าจตุรัสเอกราช (Merdeka Square) ในวันที่ 31 สิงหาคม พ.ศ.2500

ในปี พ.ศ.2506ได้มีการปรับแก้แบบธงชาติให้เป็นแบบที่ปรากฏอยู่ในปัจจุบัน โดยเพิ่มแถบแดงสลับขาวเป็น 14 แถบ และรัศมีดาราแห่งสหพันธ์เป็น 14 แฉกเพื่อแทนรัฐในสหพันธ์ทั้ง 14 รัฐ เนื่องจากมาเลเซียได้รับเอารัฐซาบาห์ รัฐซาราวัก และรัฐสิงคโปร์เข้าร่วมสหพันธ์ด้วย แม้ต่อมาสิงคโปร์จะแยกตัวเป็นเอกราชในปี พ.ศ.2508 ก็ตาม แต่ริ้วธงและรัศมีของดาราแห่งสหพันธ์ยังคงมีจำนวนอยู่เท่าเดิม เนื่องจากได้เปลี่ยนความหมายของรัฐสุดท้ายจากรัฐสิงคโปร์เป็นรัฐบาลกลางที่กรุงกัวลาลัมเปอร์ ซึ่งเป็นเมืองหลวงของประเทศ
