ตามพระราชบัญญัติการกระจายอำนาจ พ.ศ.2542 ได้มีการโอนย้ายภารกิจสู่ท้องถิ่น เพื่อให้ประชาชนได้มีโอกาสเข้ามามีส่วนร่วมในการบริหารจัดการท้องถิ่น ที่ตรงกับความต้องการของประชาชนในพื้นที่ สำหรับเป้าหมายการมีสุขภาวะที่ดีคือการที่ประชาชนสามารถดูแลสุขภาพตนเอง ครอบครัว ชุมชน และสังคมได้ด้วยตนเอง จากผลการดำเนินงานที่ผ่านมา พบว่า ประชาชนยังพึ่งตนเองด้านสุขภาพไม่ได้ อาสาสมัครสาธารณสุขชุมชน(อสม.)ซึ่งเป็นกลไกการขับเคลื่อนการดำเนินงานในชุมชนทำหน้าที่เสมือนผู้ให้บริการด้านสุขภาพในชุมชนแทน ชุมชนยังเคยชินกับการเสนอปัญหาและรอคอยรับบริการจากเจ้าหน้าที่ รูปแบบการพัฒนาคนในชุมชนของหน่วยงานภายในและภายนอกยังคงเป็นลักษณะต่างคนต่างทำ ขาดการบูรณาการ ส่งผลทำให้ปัญหาการสร้างสุขภาพและป้องกันโรคในชุมชน ไม่ได้รับการแก้ไขเท่าที่ควร การจัดทำโครงการแก้ไขปัญหาในพื้นที่ ส่วนใหญ่เจ้าหน้าที่สาธารณสุขเป็นผู้จัดทำโครงการ ซึ่งอาจไม่ตรงกับความต้องการของชุมชน ดังนั้น ความเข้มแข็งในชุมชนจึงไม่เกิดขึ้น จากสภาพปัญหาดังกล่าว คณะผู้ศึกษาจึงเกิดแนวคิดที่จะสร้างความเข้มแข็งให้กับชุมชนด้านการส่งเสริมสุขภาพและป้องกันโรค โดยเน้นกระบวนการมีส่วนร่วมของทุกภาคส่วนและใช้แผนที่ทางเดินยุทธศาสตร์ เป็นเครื่องมือในการแก้ไขปัญหาดังกล่าว วิธีการดำเนินงาน มี 6 ขั้นตอนคือ 1) การสร้างหุ้นส่วนการทำงานระดับทีมงาน 2) การสร้างสัมพันธภาพที่ดีในชุมชน 3) การสร้างสร้างหุ้นส่วนการทำงานระดับแกนนำชุมชน 4) การพัฒนาศักยภาพแกนนำชุมชน 5) การจัดการความรู้ในชุมชน 6).ประเมินผล ผลการศึกษา พบว่า ประชาชนในพื้นที่เทศบาลตำบลหาดอาษา อำเภอสรรพยา จังหวัดชัยนาท แสดงความเป็นเจ้าของชุมชน เกิดความร่วมมือในชุมชนในการแก้ไขปัญหาสุขภาพเริ่มต้นจากการเขียนโครงการเสนอคณะกรรมการกองทุนสุขภาพตำบล ทำให้เกิดนวตกรรมใหม่ๆในชุมชน เช่นสบู่ป้องกันยุง เป็นต้น และขยายผลตำบลต้นแบบ
