เอกสารประกอบการขออนุญาต
• คำขออนุญาตมีไว้ในครอบครองซึ่งวัตถุอันตรายที่กรอกข้อมูลเรียบร้อยแล้ว
• คำขอชำระค่าธรรมเนียมรายปีตาม พรบ.วัตถุอันตราย พ.ศ.2535(วอ.10) *หมายเหตุ* กรณีที่เป็นการยื่นขออนุญาตครั้งแรก
• สำเนาใบเสร็จรับเงินการชำระค่าธรรมเนียมรายปี *หมายเหตุ*กรณีที่เคยได้รับอนุญาตแล้ว
• สำเนาใบสำคัญการขึ้นทะเบียนวัตถุอันตราย(ถ้ามี)
• หนังสือมอบอำนาจ พร้อมสำเนาบัตรผู้มอบและผู้รับมอบอำนาจ(กรณี มอบหมายให้ผู้อื่นดำเนินการแทน)
• สำเนาบัตรประจำตัว/หรือสำเนาใบสำคัญประจำตัวคนต่างด้าว
• สำเนาใบอนุญาตโรงงาน (รง.4) หรือ (กนอ.)
• หนังสือสัญญาเช่าพื้นที่/โกดัง(กรณี ที่สถานที่เก็บมิใช่ของตนเอง)
• แผนผังสังเขปแสดงรายละเอียด ที่ตั้ง เส้นทางไปยังสถานที่เก็บและบริเวณข้างเคียง
• แผนผังแสดงรายละเอียดบอกขนาดความกว้าง และความยาวของอาคารที่ใช้เป็นสถานที่เก็บวัตถุอันตราย
• แผนป้องกันอุบัติภัย(Emergency plan) กรณีเกิดเหตุฉุกเฉิน
• กรณีเป็น Tank farm ต้องมีเอกสารรับรองผลการตรวจสอบความมั่นคงแข็งแรงจากผู้ประกอบวิชาชีพวิศวกรรมควบคุม
