ยอดรัก สลักใจ มีชื่อเล่นคือ แอ๊ว เกิดวันที่ 6 กุมภาพันธ์ พ.ศ. 2499 ที่ตำบลงิ้วราย อำเภอตะพานหิน จังหวัดพิจิตร เป็นบุตรนายบุญธรรม และ นางบ่าย ไพรวัลย์ มีพี่น้อง 8 คน ชาย 7 คน หญิง 1 คน โดยยอดรักเป็นคนสุดท้อง จบการศึกษา ชั้น ประถมศึกษาปีที่ 4 โรงเรียนบ้านหาดแตงโม อำเภอตะพานหิน จังหวัดพิจิตร เนื่องจากทางบ้านไม่มีเงินส่งเสียค่าเล่าเรียน[1] บิดาเสียชีวิตตั้งแต่อายุ 7 ขวบ มารดายากจนและมีพี่น้องหลายคน ได้ออกเร่ร่อนร้องเพลงที่บาร์รำวง ได้เงินคืนละ 5 - 10 บาท (ประมาณ 35-70 บาทในปัจจุบัน) ได้เงินมาก็หาซื้อหนังสือมาอ่านเอง และเรียนด้วยตนเอง จนกระทั่งได้เรียนที่โรงเรียนถาวรวิทยา อำเภอเมืองศรีสะเกษ จังหวัดศรีสะเกษ โดยเรียนการศึกษาผู้ใหญ่ และสอบเทียบจนจบชั้นมัธยมศึกษาปีที่ 3

ยอดรัก สลักใจ ได้มีโอกาสเข้าเรียนที่โรงเรียนพลตำรวจบางเขน เมื่อวันที่ 16 กันยายน พ.ศ. 2534 และศึกษาต่อจนกระทั่งปี พ.ศ. 2537 ได้รับปริญญาครุศาสตรบัณฑิตกิตติมศักดิ์ (ศศ.บ.) สาขาศิลปศาสตร์ สายดนตรีและศิลปะ การแสดง วิทยาลัยครูธนบุรี[2]

พ.ศ. 2550 ยอดรักถูกตรวจพบว่าเป็นโรคมะเร็งตับระยะแรก[3] และในวันที่ 9 สิงหาคม พ.ศ. 2551 เวลา 01.05 น.ด้วยวัย52ปี[4][5][6] ยอดรัก สลักใจ เสียชีวิตด้วยโรคมะเร็งตับที่ โรงพยาบาลพระรามเก้า กรุงเทพมหานคร หลังจากระบุว่าจะไม่ยอมรับการรักษาใดๆ อีก และไม่ต้องทำการช่วยชีวิตแม้อาการจะทรุดหนักก็ตาม โดยพิธีศพของยอดรักได้จัดขึ้นไว้ ณ ที่วัดไร่ขิง จังหวัดนครปฐม และ ทูลกระหม่อมหญิงอุบลรัตนราชกัญญา สิริวัฒนาพรรณวดี เสด็จไปในการพระราชทานเพลิงศพ ที่วัดหาดแตงโม อำเภอตะพานหิน จังหวัดพิจิตร
