ตามที่สำนักงาน The Securities and Exchange Commission (SEC) ได้กำหนดให้ บริษัทหลักทรัพย์ทุกแห่ง ต้องจัดให้ผู้ลงทุนทุกรายทำแบบประเมินความเสี่ยงในการลงทุนที่เหมาะสม (Customer Risk Profile) เพื่อให้ผู้ลงทุนได้ทราบถึงระดับความเสี่ยงที่สามารถยอมรับได้ของตนเอง ตลอดจนได้กำหนดให้ผู้ลงทุนต้องมีการทบทวนข้อมูลโดยทำแบบประเมินดังกล่าวทุกๆ 2 ปีนั้น บริษัทหลักทรัพย์กรุงศรี จำกัด (มหาชน) จึงใคร่ขอเรียบให้ทราบว่า แบบประเมินระดับความเสี่ยงในการลงทุนที่เหมาะสม (Suitability Test) ที่ท่านได้ทำในครั้งก่อนนั้น จะครบกำหนดระยะเวลาในการทบทวนในปี 2557
บริษัทฯ จึงใคร่ขอให้ท่านได้โปรดทำแบบประเมินระดับความเสี่ยงในการลงทุนที่เหมาะสม (Suitability Test) ตามที่ได้แนบมานี้ พร้อมกันนี้ได้แนบฟอร์มการแสดงตนของลูกค้า (KYC) เพื่อให้ท่านได้ทำการทบทวนและปรับปรุงข้อมูลส่วนตัวของท่านให้เป็นปัจจุบัน เพื่อประโยชน์ในการให้บริการที่ดีขึ้น โดยขอความร่วมมือให้ท่านส่งเอกสารพร้อมแนบสำเนาบัตรประชาชนที่ลงนามรับรองสำเนาถูกต้องกลับคืนมายังบริษัทฯ ภายในวันที่ 25 มีนาคม 2557 (สำหรับท่านที่ได้มีการทบทวนและปรับปรุงข้อมูลในแบบฟอร์มแสดงตนของลูกค้ากับทางผู้แนะนำการลงทุนในปี 2556 แล้ว ไม่จำเป็นต้องกรอกแบบฟอร์มดังกล่าว)
ทั้งนี้ กรณีที่ถึงกำหนดทบทวนระดับความเสี่ยงในการลงทุน แต่บริษัทฯ ไม่ได้รับการติดต่อจากท่านเพื่อทบทวนแบบประเมินความเสี่ยงในการลงทุนที่เหมาะสม บริษัทฯ จะถือว่าท่านตกลงยินยอมที่จะให้บริษัทฯ ใช้ข้อมูลการประเมินความเสี่ยงในการลงทุนที่เหมาะสมครั้งล่าสุดของท่านที่อยู่ในระบบฐานข้อมูลปัจจุบันของบริษัท ตามรายละเอียดดังต่อไปนี้ โดยมีผลจนกว่าท่านจะได้ทำแบบประเมินอีกครั้ง
บริษัทหลักทรัพย์ กรุงศรี จำกัด (มหาชน)ขอแจ้งให้ท่านทราบถึงผลการประเมิน/ทบทวนระดับความเสี่ยงในการลงทุนของท่านครั้งล่าสุดที่อยู่ในระบบฐานข้อมูลปัจจุบันของบริษัท โดยระดับคะแนนที่ท่านได้รับคือ 18 คะแนน ซึ่งแสดงว่าท่านเป็นนักลงทุนที่สามารถยอมรับความเสี่ยงในการลงทุนได้ในระดับเสี่ยงสูง ทั้งนี้ท่านสามารถศึกษารายละเอียดระดับความเสี่ยงและประเภทตราสารที่ท่านสามารถลงทุนได้ตามตารางที่แสดงในแบบประเมินระดับความเสี่ยงในการลงทุน
