มีสารโปรตีนที่ฉาบอยู่บนผิวของเม็ดโลหิตแดง ซึ่งเรียกว่า สารโปรตีน ดี (Antigen-D) เป็นตัวบ่งบอก หมู่โลหิตระบบ Rh แบ่งออกเป็น 2 หมู่ คือ
1. หมู่โลหิต Rh บวก (Rh positive) คือ หมู่โลหิตที่มีสารโปรตีน ดี (Antigen-D) อยู่ที่ผิวของเม็ดโลหิตแดง ในคนไทยมีหมู่โลหิต Rh บวก ประมาณ 99.7 % ซึ่งหมู่โลหิต Rh บวกนี้ เราเรียกง่าย ๆ ว่า “หมู่โลหิตธรรมดา”
2. หมู่โลหิต Rh ลบ (Rh negative) คือหมู่ โลหิตที่ไม่มีสารโปรตีน ดี (Antigen-D) อยู่ที่ผิวของเม็ดโลหิตแดง ในคนไทยพบว่ามีหมู่โลหิตนี้เพียง 0.3 % หรือ 1,000 คน จะพบเพียง 3 คนเท่านั้น ซึ่งเราเรียกง่ายๆ ว่า "หมู่โลหิตหายาก" หรือ "หมู่โลหิตพิเศษ" (Rh negative) นั่นเอง
