ข้าพเจ้า(ผู้ขอกู้) บริษัท แครรี่ ฯ ได้ขอสินเชื่อจากธนาคาร(ผู้ให้กู้)ธนาคารกรุงเทพ จำกัด(มหาชน) และ ข้าพเจ้มีความประสงค์ขอให้บรรษัทประกันสินเชื่ออุตสาหกรรมขนาดย่อย (บสย.) พิจารณาค้ำประกันสินเชื่อ แก่ธนาคารตามหลักเกณฑ์เงื่อนไขและวิธีการของบสย.
ข้าพเจ้าตกลงผูกพันตนตามบันทึกข้อตกลงความร่วมมือภายใต้โครงการค้ำประกันสินเชื่อ ที่ทำไว้ระหว่าง บสย. กับ ธนาคาร กับหลักเกณฑ์และวิธีปฎิบัติในการค้ำประกันสินเชื่อซึ่งแนบท้ายและเป็นส่วนหนึ่งของบันทึกข้อตกลงดังกล่าว
ข้าพเจ้าทราบดีและยินยอมว่า เมื่อธนาคารเรียกร้อง ให้ บสย. ชำระเงินตามหนังสือค้ำประกันที่ออกตามคำขอนี้ ให้อยู่ในดุลพินิจของ บสย. ที่จะพิจารณาชำระเงินตามที่เรียกร้องโดยไม่ต้องแจ้งให้ข้าพเจ้าทราบและไม่ต้องได้รับความยินยอมจากข้าพเจ้า หาก บสย. ได้ชำระหนี้สินเชื่อที่ค้ำประกันแทนข้าพเจ้าให้แก่ธนาคารไปไม่ว่าเต็มจำนวนหรือบางส่วน ข้าพเจ้าตกลงชำระคืนให้แก่ บสย.พร้อมด้วยดอกเบี้ยและค่าธรรมเนียมต่างๆ รวมทั้ง บสย. ย่อมได้รับช่วงสิทธิในทรัพย์สินหลักประกันที่ข้าพเจ้าและเจ้าของหลักประกันให้ไว้แก่ธนาคารเพื่อการบังคับชำระหนี้คืน ฉะนั้นเมื่อใดที่ข้าพเจ้าได้ชำระหนี้ทุกประเภทที่มีอยู่กับธนาคารภายใต้โครงการนี้ ตลอดจนหนี้สินอื่นเสร็จสิ้นแล้ว แต่ข้าพเจ้ายังคงมีภาระหนี้ต่อ บสย. ข้าพเจ้าและเจ้าของหลักประกัน บจ.แครรี่ ฯ
ตกลงยินยอมให้ธนาคารส่งมอบเอกสารเกี่ยวกับทรัพย์สินหลักประกันให้ บสย. ยึดถือและรักษาไว้ต่อไป
เพื่อเป็นหลักฐานในการนี้ จึงได้ลงลายมือชื่อพร้อมทั้งประทับตรา(ถ้ามี)ไว้เป็นสำคัญ
