สมเด็จย่า ทรงสนพระราชหฤทัยในคุณภาพชีวิตของพสกนิกร โดยเฉพาะอย่างยิ่งประชาชนผู้ยากไร้และด้อยโอกาสในถิ่นห่างไกลความเจริญ ด้วยทรงเห็นว่าสุขภาพคนเราเป็นสิ่งสำคัญ ถ้าสุขภาพของประชาชนดีแล้ว การประกอบอาชีพ ฐานะความเป็นอยู่ ตลอดจนการพัฒนาประเทศก็ย่อมดีตามไปด้วย
สมเด็จย่าจึงทรงบำเพ็ญพระราชกรณียกิจด้านการแพทย์ พยาบาลและการสาธารณสุข โดยทรงเจริญตามรอยยุคลบาทสมเด็จพระมหิตลาธิเบศร อดุลยเดชวิกรม พระบรมราชชนก “พระบิดาแห่งแพทย์แผนปัจจุบันและสาธารณสุข” ผู้ทรงอุทิศพระองค์และพระราชทรัพย์เพื่อกิจการด้านนี้มากมาย ดังลายพระหัตถ์เกี่ยวกับความสำคัญของการสาธารณสุขที่สมเด็จพระบรมราชชนกทรงมีถึงสมเด็จพระเจ้าบรมวงศ์เธอ กรมพระยาชัยนาทนเรนทร ว่า
“หม่อมฉันรู้สึกเสมอว่า การสาธารณสุขนั้นเป็นการสำคัญอย่างยิ่ง ทั้งเป็นเครื่องทำนุบำรุงกำลังของชาติไทย และเป็นสาธารณะประโยชน์แก่มนุษยชาติทั่วไปด้วย เพราะนั้น เมื่อมีโอกาสอันใดซึ่งหม่อมฉันพอที่จะช่วยออกกำลังกายและสติปัญญาหรือทรัพย์ อันเป็นผลที่จะทำนุบำรุงให้การนั้นเจริญขึ้นแล้ว หม่อมฉันยินดีปฏิบัติเสมอ”
ดังนั้น เมื่อสิ้นสมเด็จพระบรมราชชนกแล้ว แม้ว่าการอภิบาลพระโอรสธิดาให้มีพลานามัยและการศึกษาที่ดีจะเป้นพระราชภารกิจที่ต้องให้เวลาและความเอาใจใส่ดูแลถี่ถ้วนอย่างมาก แต่สมเด็จย่าก็ทรงมีเวลาให้แก่การแพทย์ พยาบาลและสาธารณสุขเสมอ โดยเฉพาะโครงการต่างๆ ที่สมเด็จพระบรมราชชนกเคยสนับสนุนมาแต่เดิม สมเด็จย่าทรงดำเนินกิจกรรมทุกอย่างให้เป็นไปตามพระราชประสงค์ของสมเด็จพระบรมราชชนกโดยมิได้ทรงหยุดยั้ง ผู้ใดที่เคยได้รับพระราชทานทุนการศึกษาจากสมเด็จพระบรมราชชนก ก็ทรงพระเมตตาพระราชทานต่อไปจนสำเร็จการศึกษา ทรงช่วยในการก่อสร้างปรับปรุงโรงพยาบาลศิริราชให้สำเร็จลุล่วงตามพระราชประสงค์ ได้พระราชทานเงินเพิ่มเป็นรายเดือนแก่แพทย์ผู้เป็นนักเรียนทุนของสมเด็จพระบรมราชชนกที่กลับมารับราชการแล้วได้เงินเดือนน้อยกว่าที่กำหนดไว้ นอกจากนี้ ได้พระราชทานเงินจำนวน 500,000 บาท ให้แก่จุฬาลงกรณ์มหาวิทยาลัยภายในกำหนด 25 ปี เพื่อตั้งเป็นทุนสำหรับเก็บดอกผลส่งคนไปศึกษาเพิ่มเติมในต่างประเทศ ต่อมาได้โอนทุนนี้ให้มหาวิทยาลัยแพทย์ คือ มหาวิทยาลัยมหิดลในปัจจุบัน
เมื่อตามเสด็จพระบาทสมเด็จพระเจ้าอยู่หัวอานันทมหิดลนิวัติประเทศไทยใน พ.ศ. 2488 สมเด็จย่าได้ทรงนำยาที่เรียกว่า PAS เข้ามาเมืองไทยเป็นครั้งแรก ยานี้ใช้ในการผลิตวัคซีนบีซีจี สร้างภูมิคุ้มกันวัณโรคให้ร่างกาย นับเป็นพระมหากรุณาธิคุณอย่างยิ่งต่อประชาชนชาวไทย มิให้ต้องเป็นเหยื่อของโรคร้ายนี้ ต่อมาใน พ.ศ. 2494 สมเด็จพระปิโยรสพระองค์เล็กได้พระราชทานพระราชทรัพย์ส่วนพระองค์สร้างตึกสำหรับผลิตบีซีจี ซึ่งเป็นยาฉีดให้แก่เด็กเพื่อป้องกันวัณโรค พระราชทานนามตึกนี้ว่า มหิดลวงศานุสรณ์
สมเด็จย่าทรงรับเป็นผู้อุปถัมภ์สมาคม มูลนิธิต่างๆ มากมาย เป็นต้นว่า ทรงเป็นประธานมูลนิธิอานันทมหิดล ซึ่งพระบาทสมเด็จพระเจ้าอยู่หัวโปรดเกล้าฯ ให้จัดตั้งขึ้นเมื่อ พ.ศ. 2502 เพื่อเป็นพระบรมราชานุสรณ์แด่สมเด็จพระบรมเชษฐาธิราช พระบาทสมเด็จพระปรเมนทรมหาอานันทมหิดล มีวัตถุประสงค์ในการคัดเลือกบุคคลส่งไปศึกษาวิชาพิเศษ ณ ต่างประเทศในสาขาวิชาต่างๆ เพื่อให้กลับมาปฏิบัติงานให้เป็นประโยชน์ต่อประเทศชาติ

ใน พ.ศ. 2503 ได้พระราชทานพระราชทรัพย์ส่วนพระองค์ก่อสร้าง “อาคารศรีสังวาลย์” ขึ้นที่โรงพยาบาลศิริราช เพื่อให้เป็นที่ตรวจรักษาและบำบัดคนพิการ
ทรงรับเป็นองค์ประธานกรรมการศิริราชมูลนิธิ ซึ่งได้จดทะเบียนเป็นนิติบุคคลเมื่อวันที่ 21 สิงหาคม 2510 โดยมีวัตถุประสงค์เพื่อนำดอกผลที่เกิดจากเงินกองทุนของมูลนิธิมาส่งเสริมกิจการต่างๆ ของคณะแพทย์ศาสตร์ศิริราชพยาบาล
ทรงเป็นองค์อุปถัมภ์ของมูลนิธิอนุเคราะห์คนพิการ
ทรงเป็นองค์อุปถัมภ์ของมูลนิธิช่วยคนโรคเรื้อนจังหวัดลำปาง ใน พ.ศ. 2505 และทรงอุปถัมภ์โรงเรียนจิตต์อารีของมูลนิธิซึ่งเป็นบ้านและโรงเรียนของลูกผู้ป่วยโรคเรื้อนที่ยังไม่ได้รับเชื้อจากพ่อแม่

แพทย์อาสาสมเด็จพระราชชนนี
เมื่อสมเด็จย่าเสด็จประพาสหัวเมืองเยี่ยมพสกนิกร และเจ้าหน้าที่ตำรวจ ทหารในท้องถิ่นทุรกันดารทั่วประเทศ นับแต่ พ.ศ. 2507 เป็นต้นมา ทำให้พระองค์ได้ทอดพระเนตรเห็นความทุกขเวทนาของราษฎรตามสถานที่เหล่านั้นยามเจ็บไข้ได้ป่วยด้วยพระองค์เอง ทรงพบว่าราษฎรส่วนใหญ่มักเจ็บป่วยด้วยโรคเกี่ยวกับระบบทางเดินอาหาร โรคพยาธิ โรคผิวหนัง ไข้ป่า ซึ่งเป็นโรคพื้นฐานทั่วไป แต่มักเสียชีวิตเพราะไม่มีบริการทางการแพทย์ในถิ่นทุรกันดาร
ในเดือนมีนาคม พ.ศ. 2512 สมเด็จย่าจึงโปรดเกล้าฯ ให้จัดตั้งหน่วยแพทย์อาสาเคลื่อนที่ในพระองค์ขึ้นที่จังหวัดเชียงใหม่เป็นแห่งแรก พระราชทานนามว่า “แพทย์อาสาสมเด็จพระราชชนนี” มีอักษรย่อว่า พอ.สว. หน่วยแพทย์อาสาเคลื่อนที่ พอ.สว. นี้ประกอบด้วยแพทย์ ทันตแพทย์ เภสัชกร พยาบาล เจ้าหน้าที่สาธารณสุขและสมาชิกสมทบอีกคณะหนึ่ง ซึ่งล้วนเป็นอาสาสมัครที่ไม่ได้รับสิ่งตอบแทนเป็นเบี้ยเลี้ยงหรือเงินเดือน เดินทางไปปฏิบัติงานในถิ่นทุรกันดารในวันเสาร์หรือวันอาทิตย์ โดยรถยนต์ เรือหรือเฮลิคอปเตอร์ เป็นการประจำ เพื่อตรวจรักษาราษฎรที่เจ็บไข้ได้ป่วยและแนะนำให้ความรู้เรื่องการอยู่อย่างถูกสุขลักษณะแก่ประชาชนทั่วไป
