งานระบาดวิทยา กลุ่มงานเวชกรรมสังคม โรงพยาบาลลำพูน ได้รับแจ้งจากเรือนจำจังหวัดลำพูน เมื่อวันที่ ๑ พฤศจิกายน ๒๕๕๔ ว่าพบผู้ป่วยโรคตาแดงรับการรักษาที่เรือนพยาบาล เรือนจำจังหวัดลำพูน จำนวน ๑๘๓ ราย ทีมเฝ้าระวังสอบสวนเคลื่อนที่เร็ว (SRRT) อำเภอเมือง จึงได้ดำเนินการสอบสวนโรคและควบคุมป้องกันโรค โดยมีวัตถุประสงค์เพื่อยืนยันการระบาดของโรค ค้นหาเชื้อที่เป็นสาเหตุของการระบาด หาแนวทางในการควบคุม และป้องกันโรคไม่ให้แพร่ระบาดต่อไป โดยการใช้วิธีการศึกษาทางระบาดวิทยาเชิงพรรณนา การศึกษาทางห้องปฏิบัติการ และการศึกษาทางด้านสภาพแวดล้อม ผลการสอบสวน พบผู้ป่วยเพิ่มเติมอีก ๒๗๕ ราย รวมผู้ป่วยทั้งหมด ๔๕๘ ราย โดยแยกเป็นเพศชายทั้งหมด แยกเป็น ผู้ป่วยต้องขัง แดน ๒ จำนวน ๔๓๗ ราย (ร้อยละ๙๕.๔๑) แดน ๕ จำนวน ๑๘ ราย (ร้อยละ ๓.๙๓) และผู้คุม จำนวน ๓ ราย (ร้อยละ ๐.๖๖) กลุ่มอายุที่พบผู้ป่วยมากที่สุด คือ ๒๕ – ๒๙ ปี คิดเป็นร้อยละ ๓๒.๗๕ อาการป่วยที่พบมากที่สุด คือ ตาแดง ร้อยละ ๙๙.๗๘ รองลงมา คือ ขี้ตา คิดเป็นร้อยละ ๙๙.๓๔ ซึ่งเกิดหลังจากที่มีการรับย้ายนักโทษจากเรือนจำกลางจังหวัดเชียงใหม่ มาเรือนจำจังหวัดลำพูน จำนวน ๖๗ ราย ในวันที่ ๑๘ ตุลาคม ๒๕๕๔ และจากการคัดกรองผู้ต้องขังที่รับย้ายของเจ้าหน้าที่เรือนพยาบาล พบว่า ผู้ต้องขังป่วยเป็นตาแดง จำนวน ๒ ราย ต่อมาหลังจากนั้นเป็นเวลา ๘ วัน เริ่มพบผู้ป่วยตาแดงตั้งแต่วันที่ ๒๖ ตุลาคม ๒๕๕๔ มีจำนวนผู้ป่วย ๔๔ คน หลังจากนั้นยอดผู้ป่วยก็ยังคงเพิ่มขึ้นเรื่อยๆ และพบผู้ป่วยรายสุดท้าย คือ วันที่ ๑๔ พฤศจิกายน ๒๕๕๔ จำนวน ๑ ราย การระบาดในครั้งนี้มีลักษณะปัจจัยเสี่ยงร่วม (Common source) โดยผู้ต้องขังอยู่กันอย่างแออัด และไม่สามารถปฏิบัติตามสุขอนามัยขั้นพื้นฐานได้ จึงทำให้เกิดการแพร่กระจายของโรคได้ง่ายขึ้น ส่วนการ Swab ตา ผลการตรวจวิเคราะห์ทางห้องปฏิบัติการพบเชื้อไวรัส Coxsackie A๒๔ จำนวน ๒ ราย จากผู้ป่วยทั้งหมด ๕ ราย การดำเนินการควบคุมโรค ทีมสอบสวนเคลื่อนที่เร็ว (SRRT) อำเภอเมือง ได้ให้สุขศึกษาเรื่องโรคตาแดงแก่ผู้ต้องขังและเจ้าหน้าที่ของเรือนจำจังหวัดลำพูน วางแนวทางการดูแลด้านรักษาพยาบาลร่วมกับพยาบาลอนามัยเรือนจำ รพ.สต.ริมปิง และเจ้าหน้าที่จากกองสาธารณสุขเทศบาลริมปิงและเฝ้าระวังจนผู้ป่วยหายเป็นปกติ และสถานการณ์โรคสงบไม่พบผู้ป่วยรายใหม่เพิ่มเติม
