Under LMG anesthesia, the patient was placed on the operating table in supine position. Pneumatic tourniquet was applied on left thigh. Left leg was disinfected and draped as usual. After exsanguination, one longitidunal skin incision about 4 cm was made 2 cm medially to tibial tuberosity. The wound was deepened to open the satorious fascia and the hamistring tendons (semitendinosus and gracillis) were identified. Then, the hamstring tendon were harvested by the tendon stripper. The harvested tendons were prepared with bilateral ends baseball-line suture with No.2 Ethibond about 3 cm in length. Semitendinosus 19cm folded into 9.5cm. Another stab incision was made 1cm lateral and superior to the upper pole of the patella for infusion of the N/S solution. Another stab incision was made 1cm lateral and inferior to the lower pole of the patella. The 30 degrees arthroscope was introduced into the joint cavity to check the intra-articular pathology sequentially. The intra-articular probe was inserted from the inferomedial aspect of the patella. After recording of the pathology, one shaver was inserted to clear the torn meniscus and the eroded articular surface. The loose body was than removed from the knee joint. Medial meniscus was repaired with inside out technique and Fastfix x2. The tibial tunnel guide pin is positioned by referencing a point of ACL insertion with the ACL aimer guide. The tibial tunnel is drilled to a diameter 1 to 2 mm less than the graft diameter and dilators impacted fo create the final tunnel diameter equal to the graft diameter. The femoral tunnel is created by referencing the over-the-top position with a transtibial femoral ACL drill guide to create a 1 to 2 mm or less tunnel backwall. A 2.4mm drill tip guide pin is positioned with the TTG and a cannulated headed reamer equal to the graft diameter is drilled to a depth of 50mm. The drill and guide pin are removed. The ethibond was passed through the tibial and femoral tunnel for pulling the tendon graft and endobutton. The prepared graft passing wire is hooked into the ethibond. The enthibond along with the tendon graft and endobutton was pulled proximally drawing the tendon graft through the tibial and femoral tunnels and the bioscrew was inserted into the entry hole of the tibial tunnel for fixation. Tenometer was used to measure the tension. After graft passage, a big "O" screw with washer was inserted over the proximal tibial to augment the anchor of the ethibond sutures of from the distal end of the graft. Notchplasty was performed and flexion-extension movement of the knee was done to check for ACL impingement. The wound and the knee joint were irrigated with N/S. Finally, the wounds were closed to finish the procedure.
