หากหน่วยงานกฏหมายมีความเห็นว่าพนักงานจะต้องรายงานเหตุดังกล่าวข้างต้นเป็นลายลักษณ์อักษร ให้พนักงานใช้แบบฟอร์มตามเอกสารแนบ 2 ในการรายงานข้อมูลเกี่ยวกับการร้องขอการคว่ำบาตรนั้น ทั้งนี้ เพื่อหน่วยงานกฏหมายจะได้นำส่งแบบฟอร์มตามเอกสารแนบ 2 ไปยัง ACE Global ต่อไป
