เมื่อปี 2529 องค์การอนามัยโลกได้ผลักดันแนวคิดใหม่ ออกมาอีกระลอกหนึ่ง นั่นคือ ยุทธศาสตร์การสร้างเสริมสุขภาพ (Health promotion strategy) โดยออกมาเป็นกฎบัตรออตตาวา (Ottawa charter) ขยายความ "การสร้างเสริมสุขภาพ" ออกไปจากความเข้าใจเดิมๆ ให้มีความหมายใหม่ว่า คือ "กระบวนการของการเพิ่มสมรรถนะ ให้คนสามารถควบคุมปัจจัยที่เป็นตัวกำหนดสุขภาพ และเป็นผลให้บุคคลมีสุขภาพดี สามารถควบคุมพฤติกรรมของตนเอง ให้เหมาะสม รวมไปถึงการปรับสิ่งแวดล้อม ให้เอื้อต่อการมีสุขภาพที่ดี" ซึ่งมีสาระสำคัญ 5 องค์ประกอบ คือ

การสร้างเสริมความเข้มแข็งชุมชน / กระบวนการชุมชน (Community strengthening)
การพัฒนาทักษะส่วนบุคคลให้ สามารถปรับพฤติกรรมเพื่อการสร้างสุขภาพ (Personal skill development)
การปรับเปลี่ยนระบบบริการการแพทย์ และสาธารณสุข ให้เป็นไปในลักษณะผสมผสาน และองค์รวม (Health care service system re-orientation)
