๑. การบูรณะครั้งที่ ๑ ในราว พ.ศ. ๕๐๐ โดยมีพญาสุมิตธรรมวงศา แห่งเมืองมรุกขนคร และพระอรหันต์ ๕ องค์ เป็นประธาน ในการบูรณะครั้งนั้น ได้เอาอิฐซึ่งเผาให้สุกดีแล้วมาก่อต่อเติมจากยอดพระธาตุพนมองค์เดิมให้สูง ขึ้นไปอีกประมาณ ๒๔ เมตร ( สันนิษฐานดูตามลักษณะก้อนอิฐหลังจากองค์พระธาตุพนมหักพังลงแล้ว ) แล้วอัญเชิญพระอุรังคธาตุออกจากอุโมงค์เดิม ซึ่งทำการบรรจุตั้งแต่สมัยพระมหากัสสปเถระ ขึ้นไปประดิษฐานไว้ใหม่ที่ใจกลางพระธาตุชั้นที่สอง แล้วปิดประตูอย่างมิดชิด หรือสถาปนาไว้อย่างสมบูรณ์ ( เวลานี้พบแล้ว อยู่สูงจากระดับพื้นดิน ๑๔.๗๐ เมตร )

๓. การบูรณะครั้งที่ ๓ เมื่อ พ.ศ. ๒๒๓๖ - ๔๕ โดยมีเจ้าราชครูหลวงโพนสะเม็กแห่งนครเวียงจันทน์เป็นประธาน การบูรณะครั้งนี้ ได้ใช้อิฐก่อต่อเติมจากพระธาตุชั้นที่สองซึ่งทำการบูรณะใน พ.ศ. ๕๐๐ ให้สูงขึ้นอีกประมาณ ๔๓ เมตร ได้มีการปรับปรุงที่ประดิษฐานพระอุรังคธาตุใหม่ โดยสร้างอูบสำริดครอบเจดีย์ศิลาอันเป็นที่บรรจุบุษบกและผอบพระอุรังคธาตุไว้ อย่างแน่นหนา และได้บรรจุพระพุทธรูปเงิน ทอง แก้ว มรกต และอัญมณีอันมีค่าต่าง ๆ ไว้ภายในอูบสำริดและนอกอูบสำริดไว้มากมาย มีจารึกพระธาตุพนมว่า "ธาตุปะนม" (ประนม)
