ในพ.ศ. 2491 ระหว่างทรงศึกษาอยู่ ณ ประเทศสวิตเซอร์แลนด์นั้น สมเด็จพระเจ้าอยู่หัวภูมิพลอดุลยเดช ได้ทรงขับรถยนต์ไปทรงร่วมงานที่สถานเอกอัครราชทูตไทย ณ กรุงปารีส ได้ทรงพบและมีพระราชหฤทัยสนิทเสน่หาในหม่อมราชวงศ์ สิริกิติ์ กิติยากร ธิดาของหม่อมเจ้านักขัตมงคล กิติยากร เอกอัครราชทูตไทยประจำกรุงปารีส
ในปีเดียวกันนี้สมเด็จพระเจ้าอยู่หัวทรงประสบอุบัติเหตุทางรถยนต์อย่างรุนแรงทรงบาทเจ็บที่พระพักตร์พระเนตรขวา และพระเศียรทรงเข้ารับการรักษาที่โรงพยาบาลมอร์เซส์ โปรดฯ ให้หม่อมเจ้าราชวงศ์สิริกิติ์มาเฝ้าฯ ถวายการดูแลอย่างใกล้ชิดพระสัมพันธภาพจึงแน่นแฟ้นขึ้น และต่อมาได้ทรงหมั้นหม่อมราชวงศ์ สิริกิติ์ เมื่อวันที่ 12 สิงหาคม 2492 โดยได้พระราชทานพระธำมรงค์วงที่สมเด็จพระบรมราชนกหมั้นสมเด็จพระราชชนนี
