-Motor Power = 3
-ผู้ป่วยไม่สามารถเดินและนั่งด้วยตัวเองได้
-รับประทานอาหารด้วยตัวเองไม่ได้
-ไม่สามารถปฎิบัติกิจวัติประจำวันได้ on bed rest
อภิปรายข้อวินิจฉัย
เนื่องจากผู้ป่วยได้รับการบาดเจ็บกระดูกบริเวณสะโพกและกระดูกสันหลัง ทำให้มีการปวดและชาบริเวณช่วงเอวลงไปและมีภาวะของกล้ามเนื้อแขนขาอ่อนแรง ไม่มีกำลัง ทำให้ไม่สามารถช่วยเหลือตนเองในการทำกิจวัตรประจำวันได้ หรือช่วยเหลือตนเองได้น้อย
วัตถุประสงค์
ผู้ป่วยสามารถปฏิบัติกิจวัตรประจำวันได้ด้วยตนเอง
เกณฑ์การประเมิน
-ผู้ป่วยสามารถทำกิจกรรมต่างๆได้ด้วยตนเอง
-มีอาการอ่อนเพลียลดลง
กิจกรรมการพยาบาล
1.ประเมินอาการทางระบบประสาทว่า ผู้ป่วยมีข้อจำกัดในการ เคลื่อนไหวมากน้อยเพียงใด
2.แนะนำให้ผู้ป่วยค่อยๆเพิ่มระดับกิจกรรมในแต่ละเวร เช่น เริ่มให้ผู้ป่วยนั่งห้อยเท้าบนเตียงก่อนและค่อยๆเพิ่มระดับของกิจกรรมโดยวางแผนร่วมกับผู้ป่วยและจัดให้ผู้ป่วยได้มีเวลาพักตามความเหมาะสม
3.แนะนำผู้ป่วยหากมีอาการเหนื่อย อ่อนเพลียมากขณะปฏิบัติกิจกรรมหรือออกแรง ควรหยุดทำกิจกรรมและพัก
4.ช่วยผู้ป่วยในการเคลื่อนไหว เช่น ลุกนั่ง หรือ พลิกตัว
5.ให้ผู้ป่วยออกกำลังกาย โดยหลีกเลี่ยงบริเวณที่ได้รับบาดเจ็บ เช่น หมุนข้อมือ ขยับปลายนิ้ว ยกแขน ขึ้น-ลง และช่วยผู้ป่วยแบบ Passive ROM ในส่วนที่ขยับไม่ได้แต่หลีกเลี่ยงบริเวณที่กระดูกหัก
