บันทึกข้อตกลงความร่วมมือฉบับนี้ทำขึ้นระหว่างนายฟาราฮัด อารีเยฟ และ.....................................เพื่อเป็นข้อตกลงในการดำเนินกิจการบริษัท สยาม บากู จำกัด
โดยทั้งสองฝ่ายตกลงให้ความร่วมมือกัน ดังนี้
1.ทั้งสองฝ่ายตกลงลงทุนร่วมกันในการเปิดบริษัท สยาม บากู จำกัด ในประเทศไทย ในวันที่ 2 กุมภาพันธ์ 2558 ทะเบียนเลขที่ 0105558020317 สำนักงานใหญ่เลขที่ 159/85 หมู่บ้านราชพฤกษ์ ถนนสุวินทวงศ์ แขวงแสนแสบ เขตมีนบุรี กรุงเทพมหานคร
2.ทั้งสองฝ่ายตกลงสัดส่วนในการแบ่งผลกำไร ขาดทุน ในสัดส่วนดังนี้
- นายฟาราฮัด อารีเยฟ ร้อยละ 70
- .................................. ร้อยละ 30
3.ทั้งสองฝ่ายตกลงกันให้นายฟาราฮัด อารีเยฟเป็นผู้ดำเนินกิจการบริษัท สยาม บากู จำกัด ในประเทศไทย โดยนาย.........................................สามารถตรวจสอบผลการดำเนินกิจการ งบดุลประจำปีของบริษัท ตลอดจนเอกสารหรือการทำนิติกรรมอื่นใดอันเกี่ยวข้องกับบริษัท สยาม บากู จำกัด

ทั้งนี้ทั้งสองฝ่ายตกลงจะประสานความร่วมมือระหว่างกัน และจะสนับสนุนการดำเนินงานตลอดจนการติดตาม การประเมินผลการดำเนินงาน ตามบันทึกความเข้าใจให้เกิดประโยชน์ต่อไป
บันทึกข้อตกลงนี้ จัดทำขึ้นเป็น ๒ ฉบับ มีข้อความถูกต้องตรงกัน ทุกฝ่ายได้อ่านทำความเข้าใจตามข้อตกลงโดยละเอียดแล้ว จึงได้ลงลายมือชื่อไว้เป็นสำคัญต่อหน้าพยานและต่างยึดถือไว้ ฝ่ายละ ๑ ฉบับ
