เอกสารประกอบการพิจารณาตามประเภทของกิจการ/นายจ้าง

1. สถานประกอบการ

1.4 สำเนาใบอนุญาตทำงานของนายจ้าง ถ้านายจ้างเป็นคนต่างด้าว และหากนายจ้างผู้นั้นไม่ได้ทำงานในประเทศไทยและไม่มีใบอนุญาตทำงานต้องให้ Notary Public และสถานทูตไทยรับรองการมอบอำนาจให้กรรมการหรือบุคคลใดบุคคลหนึ่งลงนามแทน
1.5 กรณีนายจ้างประกอบกิจการที่ต้องขออนุญาต ตามกฎหมายอื่น หรือต้องมีหนังสืออนุญาตให้ดำเนินกิจการจากหน่วยงานของรัฐที่เกี่ยวข้อง ให้แนบหลักฐานที่เกี่ยวข้อง เช่น หนังสืออนุญาตประกอบกิจการโรงงาน , หนังสืออนุญาตประกอบกิจการโรงแรม , ใบอนุญาตจากองค์การอาหารและยา , ใบอนุญาตให้ประกอบธุรกิจนำเที่ยว , ใบอนุญาตให้ใช้สถานที่เป็นสถานที่จำหน่ายอาหาร ฯลฯ
