โดยหนังสือมอบอำนาจฉบับนี้ บริษัท บีทีแอลซานเกียวโกลบอล จำกัด โดย Mr. Shuichi Shigyo and Mr. Seidai Sukuta กรรมการผู้มีอำนาจกระทำการ ตามหนังสือรับรอง เลขที่ สจ.4104888 สำนักงานตั้งอยู่เลขที่ 127/32 อาคารปัญจธานีทาวเวอร์ ชั้นที่ 27 ถนนนนทรี แขวงช่องนนทรี เขตยานนาวา . . กรุงเทพมหานคร ซึ่งต่อไปนี้จะเรียกว่า “ผู้มอบอำนาจ” ขอมอบอำนาจให้บุคคลผู้มีรายชื่อดังจะได้กล่าวต่อไปในหนังสือฉบับนี้ ซึ่งต่อไปนี้หากมิได้กล่าวถึงบุคคลใดโดยเฉพาะเจาะจงจะรวมกันและแทนกันเรียกว่า“ผู้รับมอบอำนาจ” เป็นผู้รับมอบอำนาจ เพื่อกระทำการแทนและในนามของผู้มอบอำนาจ โดยมีเงื่อนไขตามที่ระบุไว้ในหนังสือฉบับนี้ และประทับตราของผู้มอบอำนาจ เพื่อกระทำการ ติดต่อ และ/หรือ รับรอง ลงนามในนิติกรรมประกอบธุรกรรมต่างๆ เอกสารและหลักฐานอื่นใดประกอบธุรกรรมซึ่งต้องทำและ/หรือพึงให้ไว้กับ ธนาคารยูโอบี จำกัด (มหาชน) ซึ่งต่อไปนี้จะเรียกว่า “ธนาคาร” ในกรณีดังจะกล่าวต่อไปนี้
ข้อ 1. ในการติดต่อเกี่ยวกับบัญชีเงินฝาก ซึ่งผู้มอบอำนาจเปิดบัญชีไว้กับธนาคาร
1.1 บัญชีกระแสรายวันเลขที่ 789-363-001-9 ณ สาขา รัชดา-สาธุประดิษฐ์ .
1.2 บัญชีออมทรัพย์ เลขที่ 789-163-008-9 ณ สาขา รัชดา-สาธุประดิษฐ์ .
1.3 บัญชีเงินฝากประจำเลขที่ 789-063-086-7 ณ สาขา รัชดา-สาธุประดิษฐ์ .
ไม่ว่าจะเป็นการนำเงินเข้าบัญชี ฝากเงิน สั่งจ่าย เบิก และ/หรือถอนเงินจากบัญชี และ/หรือปิดบัญชีดังกล่าวข้างต้น รวมทั้งแจ้งอายัด ระงับการจ่ายเงิน ถอนอายัดไม่ว่าทั้งหมดหรือแต่บางส่วน ขอซื้อเช็ค สลักหลังเช็ค หรือตั๋วเงินประเภทใด ๆ อันจะพึงมีหรือจำเป็นเกี่ยวข้องกับการบัญชีดังกล่าว ผู้มอบอำนาจขอมอบอำนาจให้บุคคลดังรายนามที่จะกล่าวต่อไปนี้เป็น “ผู้รับมอบอำนาจ” โดยให้มีอำนาจภายใต้เงื่อนไขตามที่ได้ระบุไว้

ข้อ 3. นอกจากนี้ให้ผู้รับมอบอำนาจมีอำนาจอื่นๆ อีกเท่าที่จำเป็นเพื่อให้ปฏิบัติสอดคล้องกับ ระเบียบ วิธีการ มาตรฐานข้อบังคับของธนาคาร เพื่อให้การกระทำและการปฏิบัติตามวัตถุประสงค์ของหนังสือมอบอำนาจฉบับนี้สำเร็จลุล่วงไป
ข้อ 4. บรรดาการกระทำใด ๆ การใด ๆ ที่ผู้รับมอบอำนาจได้กระทำไปแล้วตามความในหนังสือมอบอำนาจฉบับนี้ ผู้มอบอำนาจยอมรับผูกพันเป็นการกระทำของผู้มอบอำนาจเองทุกประการ โดยให้มีผลใช้บังคับตั้งแต่วันที่ลงในหนังสือฉบับนี้เป็นต้นไป และแม้ต่อไปภายหน้าจะมีการเปลี่ยนแปลงกรรมการผู้ลงนามมอบอำนาจตามที่ปรากฎในหนังสือมอบอำนาจฉบับนี้ไม่ว่าท่านใดและ/หรือทั้งหมดก็ตาม ให้ถือว่าหนังสือมอบอำนาจฉบับนี้ยังคงมีผลผูกพันผู้มอบอำนาจตลอดไปจนกว่าธนาคารจะได้รับหนังสือแจ้งยกเลิกเพิกถอนหนังสือมอบอำนาจฉบับนี้เป็นประการอื่นจากผู้มอบอำนาจ
ข้อ 5. หากมีความเสียหายใดๆอันจะพึงเกิดแก่ผู้มอบอำนาจและ/หรือธนาคารจากการกระทำของผู้รับมอบอำนาจ ผู้มอบอำนาจจะไม่เรียกร้องค่าเสียหายใดๆ จากธนาคาร และจะเป็นผู้รับผิดชดใช้ค่าเสียหายทั้งจำนวน(ถ้ามี)ให้แก่ธนาคารแต่ผู้เดียวด้วย
เพื่อเป็นหลักฐาน ผู้มอบอำนาจ และผู้รับมอบอำนาจ จึงได้ลงลายมือชื่อ และประทับตราสำคัญ(ถ้ามี)ให้ไว้เป็นสำคัญต่อหน้าพยาน ณ วัน เดือน ปีที่ระบุข้างต้น
