ธงนี้มีชื่อเรียกในภาษาลาวว่า ธงดวงเดือน ได้รับการออกแบบขึ้นในช่วงสงครามโลกครั้งที่ 2 โดย มหาสิลา วีระวงส์ นักปราชญ์ผู้ยิ่งใหญ่ของชาวลาว
- สีแดง หมายถึง เลือดแห่งการต่อสู้เพื่ออิสรภาพของชาวลาว
- สีน้ำเงิน หมายถึง ความมั่งคั่งอุดมสมบูรณ์ของชาติ
- พระจันทร์สีขาว เป็นสัญลักษณ์ของดวงจันทร์ลอยเด่นเหนือลำน้ำโขง และหมายถึงเอกภาพของชาติภายใต้การปกครองของรัฐบาลพรรคประชาชนปฏิวัติลาว (พรรคคอมมิวนิสต์ลาว)หรือหมายถึงการกลับมารวมกันอีกครั้งของชาวลาวสองฝั่งโขง
 
ธงชาติลาวนี้เป็นหนึ่งในธงของประเทศที่ปกครองด้วยรัฐบาลคอมมิวนิสต์ ที่ไม่มีสัญลักษณ์รูปค้อนเคียวของขบวนการคอมมิวนิสต์สากล ซึ่งมีอยู่น้อยมาก

ในอดีตช่วงปี พ.ศ.2488นั้น ภายหลังจากญี่ปุ่นยอมแพ้สงครามโลกครั้งที่ 2แล้ว ขบวนการลาวอิสระได้ประกาศให้ประเทศลาวเป็นเอกราชจากฝรั่งเศสและประกาศจัดตั้งรัฐบาลลาวอิสระ โดยใช้ธงนี้เป็นสัญลักษณ์ในการเคลื่อนไหว แต่ 6เดือนให้หลังต่อมาฝรั่งเศสก็กลับเข้ามาปกครองลาวซึ่งเป็นส่วนหนึ่งของอินโดจีนฝรั่งเศสและล้มรัฐบาลลาวอิสระลง (ได้ให้เอกราชแก่ลาวภายหลังในปี พ.ศ.2498 ลาวจึงปกครองตนเองต้วยระบอบประชาธิปไตยอันมีพระมหากษัตริย์เป็นประมุข) ธงนี้ก็ได้ถูกนำมาใช้เป็นสัญลักษณ์ของขบวนการปะเทดลาวแทน ซึ่งขบวนการนี้จัดตั้งโดยผู้นำขบวนการลาวอิสระส่วนหนึ่ง ที่นิยมแนวคิดทางการเมืองฝ่ายซ้าย ภายใต้การนำของเจ้าสุภานุวงศ์
เมื่อขบวนการปะเทศลาวเคลื่อนไหวปลดปล่อยลาว ล้มล้างรัฐบาลระบอบกษัตริย์ และจัดตั้งรัฐบาลพรรคคอมมิวนิสต์ปกครองประเทศสำเร็จ ธงนี้ก็ได้รับการรับรองให้เป็นธงชาติของสาธารณรัฐประชาธิปไตยประชาชนลาวอย่างเป็นทางการในวันที่ 2 ธันวาคม พ.ศ.2518
