แต่ก่อนที่จะมาเป็นตลาดวังหลังแหล่งเที่ยวแหล่งกินของใครหลายคนนั้น ดั้งเดิมแล้วเมื่อครั้งอดีตเป็นส่วนหนึ่งของพระราชวังบวรสถานพิมุข หรือ วังหลัง พระราชวังของสมเด็จเจ้าฟ้าฯ กรมพระอนุรักษ์เทเวศร์ ในสมัยพระบาทสมเด็จพระพุทธยอดฟ้าจุฬาโลก และในเวลาต่อมาได้มีการจัดสร้างโรงเรียนสตรีแห่งแรกของประเทศ ชื่อว่า "โรงเรียนกุลสตรีวังหลัง" และได้ย้ายไปอยู่ที่ซอยวัฒนา ถนนสุขุมวิท (ปัจจุบันคือโรงเรียนวัฒนาวิทยาลัย) ซึ่งในเวลาต่อมา สมเด็จเจ้าฟ้าฯ กรมหลวงสงขลานครินทร์ (สมเด็จพระมหิตลาธิเบศร อดุลยเดชวิกรม พระบรมราชชนก) ทรงจัดตั้งเป็นโรงพยาบาลศิริราชขึ้นแทน โดยขอพระราชทานที่แปลงใต้คือพระราชวังของสมเด็จเจ้าฟ้าฯ กรมพระอนุรักษ์เทเวศร์ มาเป็นพื้นที่สำหรับสร้างโรงพยาบาลและมีการขยับขยายจนเต็มพื้นที่วังและถนนที่ใช้เดินทางเข้าวังนั้นจึงเรียกว่า “ถนนวังหลัง” ในส่วนตรงข้ามโรงพยาบาลนั้นกลายมาเป็นตลาดวังหลังและเป็นที่รู้จักของผู้คนในเวลาต่อมาจนถึงปัจจุบัน
