วัตถุประสงค์

-ผู้ป่วยและญาติสามารถอธิบายและตอบคำถามเกี่ยวกับอาการ สาเหตุ และการพยาบาลของโรคที่เป็นอยู่ได้
-ผู้ป่วยสามารถปฏิบัติตามคำแนะนำได้ถูกต้อง
-ผู้ป่วยสามารถปฏิบัติตัวได้ถูกต้อง
-ลดความวิตกกังวล และมีความรู้ความสามารถขณะอยู่ที่โรงพยาบาล
เกณฑ์การประเมิน
มีสีหน้าสดชื่น และสามารถตอบคำถามได้ ในเรื่องเกี่ยวกับโรคที่เป็น สาเหตุของการเกิดโรค และการดูแลตนเองเมื่อกลับบ้าน
กิจกรรมการพยาบาล
1.ประเมินความรู้ความเข้าใจของผู้ป่วยโดยการถามคำถามและใช้คำที่เหมาะสมกับผู้ป่วยเพื่อช่วยให้ผู้ป่วยเข้าใจง่ายมากขึ้น
2.ให้ความรู้เกี่ยวกับอาการ สาเหตุ และการพยาบาลให้ผู้ป่วยฟังและหลังจากอธิบายถามคำถามผู้ป่วยตามความเหมาะสม
3.เปิดโอกาสให้ผู้ป่วยและญาติได้ซักถามถึงปัญหาและข้อข้องใจต่างๆ
4.ให้ความสนใจรับฟังปัญหาของผู้ป่วยด้วยท่าทีเต็มใจเป็นกันเองและให้ผู้ป่วยได้ระบายความรู้สึกโดยไม่เอาความคิดเห็นส่วยตัวไปตัดสินปัญหาของผู้ป่วย
5.แจ้งและอธิบายให้ผู้ป่วยทราบก่อนจะให้การพยาบาลอธิบายเหตุผลและวิธีปฏิบัติตัวแก่ผู้ป่วย
6.ประเมินความรู้และการปฏิบัติตนของผู้ป่วย
7.แนะนำให้รับประทานยาให้ตรงตามเวลาและรับประทานอย่างต่อเนื่อง
8.รับประทานอาการให้ครบ 5 หมู่
-รับประทานอาหารงดไขมัน ไม่ควรรับประทานนม เนย น้ำมัน ครีม เนื้อหมู และอาหารทอดทุกชนิด
-รับประทานอาหารที่มีโปรตีนสูง จำพวกเนื้อแต่ไม่ใช้การทอด พวกแป้งน้ำตาล ผักและผลไม้ทุกชนิดลดการกินเนื้อสัตว์
9.การพักผ่อนและการออกกำลังกาย พักผ่อนอย่างนอนวันละ 6 – 8 ชั่วโมง และมีการออกกำลังพอสมควรตามสภาพ เช่นการเดินวันละ 15 – 30 นาที
10.แนะนำให้สังเกตแผลว่ามีอาการ ปวด บวม แดง ร้อน หรือมีหนองซึมที่บริเวณแผลและให้แจ้งพยาบาลทันที
