เมื่อปี ๒๕๐๕ นายเปรม อักษรสว่าง สมาชิกสภาจังหวัดพัทลุง และนายกพุทธิกสมาคมบางแก้ว ได้นำสมาชิกฯ ยุวกสิกรและประชนประมาณ ๕๐๐ คน ขุดลอกเหมืองส่งน้ำจากคลองตะโหมดหรือคลองท่าเชียดเพื่อนำไปใช้ในเขตอำเภอเขาชัยสน จังหวัดพัทลุง แต่ไม่ได้ผล เนื่องจากจะต้องขุดเหมืองส่งน้ำลึกประมาณ ๕-๖ เมตร ซึ่งเกินกำลังที่เกษตรกรจะทำได้ ต้องใช้เทคนิคและเครื่องจักรเครื่องมือในการดำเนินงาน จึงร้องเรียนผ่านชลประทานภาคใต้ในสมัยนั้น ขอให้พิจารณาเปิดโครงการท่าเชียดขึ้นเพื่อช่วยเหลือพื้นที่เพาะปลูกดังกล่าวชลประทานภาคใต้ได้ส่งเจ้าหน้าที่ไปพิจารณาเห็นว่าลู่ทางที่จะเปิดการก่อสร้างโครงการฯ ขึ้นได้จึงส่งเจ้าหน้าที่ออกไปทำการสำรวจรายละเอียดภูมิประเทศและสถิติต่างๆ เพื่อนำมาวางโครงการและออกแบบ และได้ก่อสร้างโครงการเสร็จสิ้นในปี พ.ศ.๒๕๑๔ มีพื้นที่ทั้งหมด ๑๒๑,๕๒๗ ไร่ พื้นที่ชลประทาน ๑๐๐,๐๐๐ ไร่

วันที่ ๑๔ กันยายน ๒๕๒๐ พระบาทสมเด็จพระเจ้าอยู่หัวได้มีพระราชดำริให้กรมชลประทานพิจารณาโครงการชลประทานเพื่อช่วยเหลือการทำนาในเขตจังหวัดพัทลุง ต่อมาสำนักงานชลประทานที่ ๑๒ (ปัจจุบันคือสำนักชลประทานที่ ๑๖) ได้รับหนังสือจากอำเภอตะโหมด ที่ พท.๐๖๑๖/๒๕๖ ลงวันที่ ๒๕ มกราคม ๒๕๓๒ ขอให้พิจารณาความเป็นไปได้โครงการอ่างเก็บน้ำโละหนุน ตามพระราชดำริ เพื่อช่วยเหลือพื้นที่เพาะปลูกในเขตโครงการชลประทานท่าเชียด จำนวน ๑๐๐,๐๐๐ ไร่ และลดอันตรายจากอุทกภัยในเขต อำเภอตะโหมด อำเภอเขาชัยสน จังหวัดพัทลุง

ประธานสภาตำบลตะโหมด (นายบุญชอบ เพชรหนู) ประธานสภาตำบลคลองใหญ่ (นายหวัง เสถียร) และประธานตำบลแม่ขรี (นายสวัสดิ์ มุสิกะสงค์)ได้มีหนังสือ ลงวันที่ ๖ กรกฎาคม ๒๕๓๓ เรียนราชเลขาธิการ ขอพระราชทานสร้างอ่างเก็บน้ำคลองหัวช้าง โดยให้เหตุผลว่าท้องที่ตำบลตะโหมด ตำบลคลองใหญ่ ตำบลแม่ขรี อำเภอตะโหมด จังหวัดพัทลุง รวม ๒๕ หมู่บ้านประชากร ๒๑,๔๔๓ คน มีการทำนาทำสวนเป็นอาชีพหลัก เนื้อที่การเกษตร ๗๓,๑๕๘ ไร่ แต่การประกอบอาชีพดังกล่าวไม่ได้ผลเต็มที่ เนื่องจากขาดแคลนน้ำเพื่อการเกษตร และเป็นคลังจ่ายน้ำให้กับโครงการชลประทานท่าเชียด

ต่อมาราชเลขาธิการ ได้มีหนังสือ ที่ รล ๐๐๕/๑๐๒๐๖ ลงวันที่ ๒๖ กรกฎาคม ๒๕๓๓ เรียนอธิบดีกรมชลประทาน และหนังสือที่ รล ๐๐๐๕/๑๐๒๐๗ ลงวันที่ ๒๖ กรกฎาคม ๒๕๓๓ เรียนผู้ว่าราชการ จังหวัดพัทลุง แจ้งให้ทราบถึงความประสงค์ของราษฎรที่ต้องการให้ก่อสร้างอ่างเก็บน้ำคลองหัวช้าง จึงขอให้พิจารณาในเรื่องนี้ และหากมีความคิดเห็นหรือข้อเสนอแนะประการใด แจ้งให้สำนักราชเลขาธิการทราบด้วย

จากการประสานงานระหว่างกรมชลประทาน โดยสำนักชลประทานที่ ๑๒ (ปัจจุบันคือสำนักชลประทานที่ ๑๖) กับจังหวัดพัทลุง เห็นว่ามีความเป็นไปได้ที่จะดำเนินการในเรื่องนี้ และได้ดำเนินการพิจารณาเบื้องต้นแล้วเสร็จเมื่อเดือนตุลาคม ๒๕๓๓

สำนักชลประทานที่ ๑๒ ได้มีบันทึกที่ กษ ๐๓๓๗/๓๕๐ ลงวันที่ ๑๘ ตุลาคม ๒๕๓๓ เรียนอธิบดีกรมชลประทาน เห็นสมควรขอพระราชทานโครงการเป็นโครงการพระราชดำริ

กรมชลประทานได้มีหนังสือที่ กษ ๐๓๐๑/๑๘๒๕ ลงวันที่ ๑๑ มีนาคม ๒๕๓๔ เรียนราชเลขาธิการ เพื่อนำความกราบบังคมทูลพระกรุณารับเป็นโครงการอันเนื่องมาจากพระราชดำริ

สำนักราชเลขาธิการ ได้มีหนังสือที่ รล ๐๐๐๕/๕๒๒๖ ลงวันที่ ๑๘ เมษายน ๒๕๓๔ และหนังสือที่รล ๐๐๐๕/๕๒๒๗ ลงวันที่ ๑๘ เมษายน ๒๕๓๔ แจ้งให้กรมชลประทานและผู้ว่าราชการจังหวัด ทราบตามลำดับ ว่าความทราบฝ่าละอองธุลีพระบาทแล้ว ทรงพระกรุณาโปรดเกล้าฯ รับเป็นโครงการอันเนื่องมาจากพระราชดำริ
