“ผู้ให้บริการ” ต้องจัดทำรายงานการซ่อมแซม/บำรุงรักษา ให้ “ผู้รับบริการ” หรือผู้แทนลงลายมือชื่อในรายงานการ เพื่อเป็นหลักฐานทุกครั้งที่มีการซ่อมแซม/บำรุงรักษา
