หนังสือสัญญาฉบับนี้ทำขึ้นระหว่าง บริษัท ออโตลิฟ (ประเทศไทย) จำกัด โดย นางมณี แบกกิวลี่ กรรมการผู้มีอำนาจ สำนักงาน ตั้งอยู่ บ้านเลขที่ 700/415 หมู่ที่ 7 ตำบล ดอนหัวฬ่อ อำเภอเมืองชลบุรี จังหวัดชลบุรี ซึ่งต่อไปนี้ในสัญญาเรียก ‘’นายจ้าง’’ ฝ่ายหนึ่ง กับ นายภูมินทร์ สมมาตร์ ซึ่งต่อไปนี้เรียก ‘’ลูกจ้าง’’ อีกฝ่ายหนึ่ง ทั้งสองฝ่ายได้ตกลงทำสัญญานี้ไว้ต่อกัน ดังมีข้อความต่อไปนี้ คือ
ข้อ 1 . นายจ้างตกลงส่งลูกจ้างไปฝึกงานตำแหน่ง ช่างเทคนิค ณ ประเทศ เม็กซิโก มีกำหนดเวลา 21 วัน โดยลูกจ้างจะเริ่มฝึกงานในตำแหน่งดังกล่าวตั้งแต่วันที่ 4 กันยายน 2559 จนถึงวันที่ 24 กันยายน 2559
ข้อ 2. ลูกจ้างจะต้องมาฝึกงาน ณ สถานที่ฝึกงานเวลา 08.00 – 17.00 น. พักเวลา 12.00 – 13.00 น. โดยมีวันหยุดประจำสัปดาห์ คือวันเสาร์และวันอาทิตย์และวันหยุดประเพณีประเทศเม็กซิโก
ข้อ 3. นายจ้างตกลงจ่ายค่าจ้างในประเทศไทย และเบี้ยเลี้ยงระหว่างฝึกงานในต่างประเทศให้แก่ลูกจ้าง ณ สถานที่ฝึกงานของลูกจ้างในต่างประเทศ/สำหรับค่าจ้างจ่ายเข้าบัญชีเงินฝากของลูกจ้างในประเทศไทยภายในวันที่ 27 ของเดือนตามอัตราดังต่อไปนี้
1. ค่าจ้างเดือนละ 22,450 บาท ( สองหมื่นสองพันสี่ร้อยห้าสิบบาทถ้วน)
2. เบี้ยเลี้ยง เดือนละ 20,000 บาท (สองหมื่นบาทถ้วน)
ข้อ 4. นอกเหนือจากค่าจ้าง เบี้ยเลี้ยงข้อ 3 แล้ว นายจ้างตกลงจัดที่พักที่เหมาะสมและอาหารวันละ 3 มื้อ ให้ลูกจ้างด้วย
ข้อ 5. นายจ้างตกลงเป็นผู้รับผิดชอบค่าใช้จ่ายของลูกจ้างในการเดินทางไปฝึกงานต่างประเทศและระหว่างฝึกงานอยู่ต่างประเทศตามสัญญามีดังต่อไปนี้
1. ค่าธรรมเนียมในการทำหนังสือเดินทางและค่าธรรมเนียมในการขอวีซ่า
2. ค่าเตรียมตัวเดินทางเป็นจำนวนเงิน - บาท
3. ค่าโดยสาร ไป - กลับ โดยทางบริษัทเป็นผู้ออกให้
4. ค่ารักษาพยาบาลตลอดระยะเวลาที่ฝึกงานอยู่ต่างประเทศ
ข้อ 6. ลูกจ้างจะต้องปฏิบัติหน้าที่ของตนด้วยความซื่อสัตย์และขยันหมั่นเพียร โดยปฏิบัติตามคำสั่งหรือข้อบังคับใดๆ ของนายจ้างหรือผู้ที่ได้รับมอบหมายจากนายจ้าง ณ สถานที่ทำงานของตน
ข้อ 7. ลูกจ้างจะต้องปฏิบัติงานด้วยความระมัดระวังเท่าที่จำเป็นเพื่อป้องกันความเสียหายอันอาจจะมีขึ้น และต้องใช้ความระมัดระวังเพื่อความปลอดภัยต่อร่างกายและทรัพย์สินของนายจ้างหรือบุคคลอื่น
ข้อ 8. นายจ้างตกลงทำประกันอุบัติเหตุและเจ็บป่วยทั้งในและนอกเวลาฝึกงานให้แก่ลูกจ้างตลอดระยะเวลาการฝึกงาน
ข้อ 9. ในกรณีภาวะสงคราม เหตุการณ์จลาจล วิกฤตทางการเมือง หรือภัยธรรมชาติในประเทศที่ลูกจ้างฝึกงานอยู่ นายจ้างตกลงเป็นผู้รับผิดชอบค่าใช้จ่ายในการอพยพลูกจ้างไปในที่ปลอดภัยหรือนำกลับประเทศไทยโดยทันที
ข้อ 10. ลูกจ้างจะต้องปฏิบัติตามกฎระเบียบ กฎหมายขนบธรรมเนียมประเพณีของประเทศที่ลูกจ้างไปฝึกงานอย่างเคร่งครัด

สัญญาฉบับนี้ทำขึ้น 4 ฉบับ ซึ่งมีข้อความตรงกันทุกประการสำหรับเจ้าหน้าที่กรมการจัดหางานจำนวน 2 ฉบับ นายจ้างและลูกจ้างไว้ฝ่ายละฉบับ คู่สัญญาทั้งสองฝ่ายได้อ่านและเข้าใจข้อความแห่งสัญญานี้โดยตลอดแล้ว จึงได้ลายมือชื่อไว้เป็นสำคัญต่อหน้าพยาน
