พระบาทสมเด็จพระเจ้าอยู่หัวทรงมีความห่วงใยประชาชนชาวไทยในทุกด้าน โดยเฉพาะในด้านสุขภาพอนามัย ซึ่งพระองค์ทรงถือว่าปัญหาด้านสุขภาพอนามัยของประชาชนนั้นเป็นปัญหาสำคัญที่ต้องได้รับการแก้ไข ดังพระราชดำรัสที่ว่า
“ถ้าคนเรามีสุขภาพเสื่อมโทรม ก็จะไม่สามารถพัฒนาชาติได้ เพราะทรัพยากรที่สำคัญของประเทศชาติ ก็คือ พลเมือง นั่นเอง”

ในการเสด็จพระราชดำเนินไปทรงเยี่ยมราษฎรตามท้องที่ต่างๆ พระองค์จะทรงพระกรุณาโปรดเกล้าฯ ให้มีคณะแพทย์ที่ประกอบด้วยผู้เชี่ยวชาญในแต่ละสาขาจากโรงพยาบาลต่างๆ และแพทย์อาสาสมัคร โดยเสด็จพระราชดำเนินไปในขบวนอย่างใกล้ชิด พร้อมด้วยเวชภัณฑ์และเครื่องมือแพทย์ครบครัน พร้อมที่จะให้การรักษาพยาบาลราษฎร ผู้ป่วยไข้ได้ทันที นอกจากนั้นพระองค์ยังทรงได้ริเริ่มหลายโครงการด้านการแพทย์และสาธารณสุข ดังนี้
โครงการหน่วยแพทย์พระราชทาน
ทรงพระกรุณาโปรดเกล้าฯให้แพทย์ประจำพระองค์ที่ตามเสด็จฯ ตรวจและรักษาคนไข้ตั้งแต่วันที่ ๒๙ มกราคม ๒๕๑๒ เมื่อเสด็จพระราชดำเนินทอดพระเนตรโครงการชาวเขา และทรงพบว่าราษฎรที่มารอรับเสด็จป่วยเป็นไข้กันมาก
โครงการดังกล่าวประกอบด้วยการบำบัดรักษาจากคณะแพทย์พระราชทาน และอบรมหมอหมู่บ้าน เป็นการช่วยแก้ไขปัญหาด้านสุขภาพอนามัยของราษฎรที่อยู่ในพื้นที่ห่างไกลและมีอุปสรรคที่ระบบปกติยากจะดูแลได้ทั่วถึง โดยเฉพาะอย่างยิ่ง ราษฎรส่วนใหญ่มีฐานะยากจนและขาดความรู้ในการดูแลรักษาตนเอง

เริ่มเมื่อ พ.ศ. ๒๕๑๐ เพื่อตรวจรักษาราษฎรในท้องถิ่นทุรกันดารโดยไม่คิดมูลค่า และถ้าจำเป็นก็จัดส่งไปยังโรงพยาบาลในจังหวัดต่างๆ ที่พระบาทสมเด็จพระเจ้าอยู่หัวเสด็จแปรพระราชฐานประทับแรม และในท้องถิ่นต่างๆ ที่ห่างไกลตัวเมืองมาก ได้ทรงพระกรุณาโปรดเกล้า ฯ ให้จัดเจ้าหน้าที่แพทย์ พยาบาล เครื่องมือเครื่องใช้ ตลอดจนยารักษาโรค ออกทำการตรวจรักษาราษฎรในท้องถิ่นกันดาร

โครงการตามพระราชประสงค์ หมายถึง โครงการที่ทรงศึกษาปฏิบัติส่วนพระองค์กับผู้เชี่ยวชาญในศาสตร์สาขาต่างๆ เมื่อได้ผลดีแล้วจึงทรงนำมาใช้ให้เกิดประโยชน์แก่ประชาชน
โดยโครงการแพทย์พิเศษตามพระราชประสงค์ เริ่มเมื่อ พ.ศ. ๒๕๑๗ เพื่อบรรเทาความเจ็บปวดของราษฎรที่นิคมสร้างตนเองพัฒนาภาคใต้ อำเภอแว้ง จังหวัดนราธิวาส นิคมนี้มีสถานีอนามัยเพียงแห่งเดียว ทรงพระกรุณาโปรดเกล้าฯ ให้กระทรวงสาธารณสุขจัดแพทย์หมุนเวียนเข้าไปบริการตรวจรักษา แพทย์และเจ้าหน้าที่จากโรงพยาบาล นราธิวาส และโรงพยาบาลสุไหงโก-ลก ออกไปปฏิบัติการสัปดาห์ละ ๒ ครั้ง เป็นประจำ
