เอกสารฉบับนี้ จัดทำที่ บริษัท เอส พี บี เอ็นจิเนียริ่ง จำกัด จัดทำขึ้นโดยระบุเงื่อนไข เบื้องต้น
ก่อนการจ้างงานจริง โดยเอกสารนี้ ยังไม่ถือว่า เป็นสัญญาจ้างงานแรงงานฉบับสมบูรณ์ โดยมีเนื้อหา ดังนี้
( ข้อที่ 1 ) ตำแหน่งงาน และ อัตราค่าจ้าง
1.1 บริษัท ฯ มีความประสงค์จะจ้าง ในตำแหน่ง วิศวกร
( ข้อที่ 2 ) อัตราค่าจ้าง และ ค่าล่วงเวลา
2.1 บริษัท ฯ มีความประสงค์จะจ่ายค่าจ้าง เงินเดือน 14,500 บาท / เดือนไม่ร่วมOT
2.2 บริษัท ฯ มีความประสงค์จะค่าสวัสดิการเป็นรายวัน ทดลองงาน 40 บ./ วัน:ผ่านทดลองงาน 80 /วัน
2.3 อัตราค่าล่วงเวลา ( OT ) จะจ่ายตาม กฏหมายแรงงานประเทศไทย
2.4 ระยะเวลาทดลองงาน 4 เดือน ( 120 วัน )
( ข้อที่ 3 ) สถานที่ทำงาน และระยะเวลาการจ้างงาน
3.1 บริษัท ฯ มีความประสงค์จะให้ท่านทำงาน ณ.156 ม.4 ต.พะยอม อ.วังน้อย จ.อยุธยา
3.2 ระยะเวลาในการจ้างงาน กฏหมายแรงงานประเทศไทย
( ข้อที่ 4 ) ชั่วโมงการทำงาน / วัน / สัปดาห์ วันหยุด
4.1 ชั่วโมงการทำงานปกติ ไม่เกิน 8 ชั่วโมง / วัน
4.2 การทำงาน ต่อ สัปดาห์ 6 วัน / สัปดาห์
4.3 กรณีที่ทำงาน วันหยุด ประจำสัปดาห์ ได้รับค่าจ้างสัปดาห์ละ 1 วัน ( วันอาทิตย์ )
4.4 บริษัทฯ จะจัดให้มีวันหยุดตามประเพณีไทยโดยได้รับค่าจ้าง ไม่ต่ำกว่า 13 วัน / ปี
( ข้อที่ 5 ) ค่าล่วงเวลาและค่าจ้างในวันหยุด
5.1 หากทำงานเกินเวลาทำงานปกติ จะจ่ายค่าล่วงเวลา( OT. ) ให้ กฏหมายแรงงานประเทศไทย
5.2 หากทำงานในวันหยุด จะ จ่ายค่าจ้างในวันหยุดให้ กฏหมายแรงงานประเทศไทย
( ข้อที่ 6 ) อาหาร บริษัท ฯ จัดอาหาร กลางวัน (กรณีที่วันทำงานปกติ) อาหารกลางวันทุกวันจันทร์ / สัปดาห์
( ข้อที่ 7 ) ที่พัก บริษัท ฯ จะจัดหาที่พักให้ โดยเป็นสวัสดิการ จัดหาห้องพักให้ฟรี
ยกเว้น ค่าน้ำ และ ค่าไฟฟ้า ต้องชำระเอง
( ข้อที่ 8 ) ค่ารักษาพยาบาล บริษัท ฯ จะขึ้นทะเบียน ประกันสังคม กฏหมายแรงงานประเทศไทย
( ข้อที่ 9 ) การเดินทาง บริษัท ฯจัดให้มีรถรับ ( ครั้งแรกเท่านั้น ) ชายแดนอรัญประเทศ- บริษัท
