สัญญานี้ทำขึ้นระหว่าง นายแพทย์สมัย ศิริทองถาวร ผู้อำนวยการสถาบันพัฒนาการเด็กราชนครินทร์ อยู่บ้านเลขที่ 196 หมู่ที่ 10 ถนน เชียงใหม่-ฝางตำบล/แขวง ดอนแก้ว อำเภอ/เขต แม่ริม จังหวัด เชียงใหม่ . ซึ่งต่อไปในสัญญานี้เรียกว่า “เจ้าของทรัพย์” ฝ่ายหนึ่ง กับ อายุ………ปี ถือบัตรประชาชนเลขที่ อยู่บ้านเลขที่……….....ตรอก/ซอย……...................……………... ถนน…………......…..……ตำบล/แขวง…………….…..อำเภอ/เขต……………….….จังหวัด……………….....… ซึ่งเป็นผู้ดูแล................................................. บัตรประชาชนเลขที่ .............................................เป็นผู้เข้าร่วมโครงการดังกล่าวข้างต้น ซึ่งต่อไปในสัญญานี้เรียกว่า “ผู้ยืม” อีกฝ่ายหนึ่ง

คู่สัญญาทั้งสองฝ่ายตกลงทำสัญญากันดังมีข้อความดังต่อไปนี้

ข้อ 1. เจ้าของทรัพย์มีโครงการจัดหาอุปกรณ์เครื่องช่วยความพิการสำหรับเด็กและคนพิการ ผู้ยืม มีความประสงค์เข้าร่วมโครงการดังกล่าวข้างต้น

เจ้าของทรัพย์ได้ส่งมอบทรัพย์ที่ยืมให้แก่ผู้ยืมในสภาพที่ใช้งานได้ตามปกติ และผู้ยืมได้รับมอบทรัพย์ที่ยืมไปถูกต้องครบถ้วนในวันทำสัญญาฉบับนี้แล้ว

ข้อ 2. หากทรัพย์ตามข้อ 1. ถูกผู้ยืมขาย จำหน่ายจ่ายโอน หรือ ให้แก่ผู้อื่น หรือกระทำการใด ๆ จนเป็นเหตุให้ผู้ยืมไม่สามารถคืนเครื่องช่วยยกผู้ป่วยแก่เจ้าของทรัพย์ได้ ผู้ยืมจะต้องรับผิดชอบโดยการจ่ายเงินคืนให้แก่เจ้าของทรัพย์เป็นจำนวนเงิน 20,000 บาท ( สองหมื่นบาทถ้วน )

ข้อ 3. ค่าใช้จ่ายต่าง ๆ เกี่ยวกับทรัพย์ที่ยืมตามสัญญาฉบับนี้ เช่น ค่าส่งคืนทรัพย์ที่ยืม ค่าดูแลรักษา ค่าซ่อมแซม เป็นต้น ผู้ยืมเป็นผู้รับภาระเองทั้งสิ้น

ข้อ 4. ผู้ยืมจะต้องใช้ทรัพย์ที่ยืมอย่างปกติแห่งการใช้ทรัพย์นั้น และผู้ยืมไม่นำเอาทรัพย์ที่ยืมออกไปให้บุคคลอื่นใช้สอย โดยปราศจากความยินยอมเป็นลายลักษณ์อักษรจากเจ้าของทรัพย์

ข้อ 5. ผู้ยืมต้องดูแลรักษาทรัพย์ที่ยืม อย่างเช่นวิญญูชนทั่วไปพึงกระทำ ในการสงวนทรัพย์สินของตน

ข้อ 6. เมื่อสัญญานี้สิ้นสุดลง ผู้ยืมต้องคืนทรัพย์แก่เจ้าของทรัพย์ในสภาพที่ดี ใช้งานได้ดังเดิมเหมือนในขณะที่ยืมไปทุกประการ

ข้อ 7. ผู้ยืมต้องรับผิดชอบในบรรดาความเสียหายที่เกิดขึ้นกับทรัพย์ที่ยืม ไม่ว่าจะเป็นการกระทำของผู้ยืม ผู้ป่วย หรือ บุคคลอื่นใดก็ตาม

สัญญานี้ทำขึ้นเป็นสองฉบับมีข้อความถูกต้องตรงกัน คู่สัญญาทั้งสองฝ่ายได้อ่านและเข้าใจดีแล้วจึงลงลายมือชื่อไว้ต่อหน้าพยานเป็นสำคัญ และเก็บสัญญาไว้ฝ่ายละฉบับ
