บทคัดย่อ : วัตถุประสงค์ของการวิจัย : เพื่อศึกษาถึงประสิทธิผลของการจัดการแบบรายกรณีโดยผู้ปฏิบัติการพยาบาลขั้นสูงในผู้ป่วยโรคความดันโลหิตสูงที่ไม่สามารถควบคุมความดันโลหิตได้
การออกแบบวิจัย : การวิจัยกึ่งทดลอง( Quasi-experiment research)
วิธีดำเนินการวิจัย : กลุ่มตัวอย่างประกอบด้วยผู้ป่วยโรคความดันโลหิตสูงที่ไม่สามารถคุมความดันโลหิตได้ มารับบริการที่คลินิกโรคความดันโลหิตสูง โรงพยาบาลบ้านโฮ่ง ตั้งแต่เดือนตุลาคม พ.ศ.2555 ถึงเดือนมีนาคม พ.ศ. 2556 จำนวน 30 คน โดยทดลองแบบหนึ่งกลุ่ม ทดสอบก่อนและหลัง เครื่องมือที่ใช้ในการศึกษา ประกอบด้วย1) แบบประเมินปัญหาแบบรายกรณี 2) คู่มือการจัดการตนเองสำหรับผู้ป่วยโรคความดันโลหิตสูง 3) สมุดบันทึกประจำตัวผู้ป่วยโรคความดันโลหิตสูง 4) แบบประเมินความรู้เรื่องโรคความดันโลหิตสูง 5) แบบประเมินพฤติกรรมสุขภาพ ดำเนินการศึกษา ด้วยการจัดการแบบรายกรณีโดยผู้ปฏิบัติการพยาบาลขั้นสูง ร่วมกับผู้ป่วยและครอบครัว 4 ครั้ง ติดตามเยี่ยมบ้าน 2 ครั้งในระยะเวลา 24 สัปดาห์ โดยใช้แบบประเมินปัญหาผู้ป่วยแบบรายกรณี เพื่อให้ผู้ป่วยได้รับรู้ถึงข้อมูลสุขภาพตามปัญหาของตนเอง ความรู้เรื่องโรคและปัญหาที่กำลังเผชิญ ภาวะแทรกซ้อน การดูแลตนเองใน 6 ด้าน ได้แก่ การรับประทานอาหาร การออกกำลังกาย การควบคุมน้ำหนัก การเลิกสูบบุหรี่ การงดดื่มสุรา การจัดการความเครียด การวัดความดันโลหิตด้วยตนเองและการบันทึก การจัดการในภาวะฉุกเฉินทางหลอดเลือดสมองและหลอดเลือดหัวใจ และทักษะการปรับเปลี่ยนพฤติกรรมสุขภาพ โดยมีการตั้งเป้าหมายร่วมกันและให้การเสริมพลัง (Empower and prepare patients to manage their health care) วิเคราะห์ข้อมูลโดยการแจกแจงความถี่ หาค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และ Pair-simples T-test
