สัญญาว่าจ้างรถรับ - ส่ง พนักงาน
สัญญานี้ทำขึ้นระหว่างบริษัท อาปิโก พลาสติค จำกัด (มหาชน) โดยนายเย็บ ซู ชวน และนางเตียว ลี งอ ตำแหน่งกรรมการบริษัท สำนักงานตั้งอยู่เลขที่ 358-358/1 หมู่ 17 นิคมอุตสาหกรรมบางพลี ตำบลบางเสาธง อำเภอบางเสาธง จังหวัดสมุทรปราการ ซึ่งในสัญญาต่อไปนี้เรียกว่า “ผู้ว่าจ้าง” ฝ่ายหนึ่ง
กับ บริษัท โดยนาย ที่อยู่เลขที่ ซึ่งในสัญญาต่อไปนี้เรียกว่า “ผู้รับจ้าง” อีกฝ่ายหนึ่ง
ทั้งสองฝ่ายได้ตกลงทำสัญญาดังมีข้อความต่อไปนี้.-

ข้อ 1. ผู้ว่าจ้างตกลงให้ผู้รับจ้าง เป็นผู้จัดการหารถตู้ปรับอากาศตามบันทึกแนบท้ายสัญญา เพื่อใช้เป็นพาหนะในการอำนวยความสะดวกในการบริการ รับ-ส่ง พนักงานของผู้ว่าจ้าง จำนวน 5 คัน พร้อมพนักงานขับรถ โดยรถบริการของผู้รับขนส่งต้องมีสภาพดี ได้มาตรฐานและมีความปลอดภัยต่อผู้โดยสาร

ข้อ 2. ผู้รับจ้างต้องนำรถยนต์ดังกล่าวมารับส่งพนักงานของผู้ว่าจ้าง ณ. ที่ทำการโรงงานของผู้ว่าจ้าง เลขที่ 358-358/1 หมู่ 17 นิคมอุตสาหกรรมบางพลี ตำบลบางเสาธง อำเภอบางเสาธง จังหวัดสมุทรปราการ วันละ 2 เที่ยวต่อ 1 คัน ให้ทันตามเวลาทำงาน และเลิกงานของบริษัท คือ 08.00 น.และ 17.00 น.ทุกวันที่โรงงานของผู้ว่าจ้างเปิดดำเนินการ เว้นแต่ผู้ว่าจ้างจะได้กำหนดให้เป็นอย่างอื่น

ข้อ 3. เส้นทางการเดินรถ ที่ผู้รับจ้างจะต้องนำรถพาหนะบริการ ตามบันทึกแนบท้ายสัญญา
