๑) บุคลิกภาพเดิมของผู้ป่วยเปลี่ยนไป เช่น เคยเป็นคนสะอาด และสุภาพ ก็เปลี่ยนเป็นสกปรก และหยาบคาย เคยเป็นคนพูดน้อย ก็กลายเป็นพูดไม่หยุด ฯลฯ

๒) ไม่อยู่ในโลกของความจริง เช่น มีอาการ ประสาทหลอน ได้ยินเสียงคนด่าตลอดเวลา ทั้งๆ ที่ ในขณะนั้นไม่มีใครด่าเขาเลย หลงผิดว่ามีคนคิดร้าย และกำลังตามฆ่าเขา
